摘要 目的:探讨^(18)F-FDG PET/MR在评价乳腺腺癌分期及复发检测中的临床应用价值。方法 :回顾性分析接受^(18)FFDG PET/MR检查的29名乳腺癌患者的资料,由2名核医学科医师对PET/MR图像进行诊断评价。采用Kappa一致性检验评价乳腺癌PET/MR分期和病理金标准的诊断一致性;采用诊断试验评价指标评估PET/MR对复发检测的应用价值。统计学软件采用SPSS 22.0。结果:^(18)F-FDG PET/MR术前临床分期与金标准的诊断一致性较高(Kappa值=0.892)。PET/MR检测复发的灵敏度为100%,特异性为92.3%,准确率为94.4%,阳性预测值为80%,阴性预测值为100%。结论:^(18)F-FDG PET/MR对乳腺癌分期及复发检测具有较高的应用价值,有望成为一种准确、无创评估乳腺癌患者病情的影像学检查方法。 Objective: To evaluate the clinical application value of^(18)F-flurodeoxyglucose(FDG) PET/MR in staging and recurrence detecting of breast cancer. Methods: Twenty-nine breast cancer patients who underwent PET/MR were retrospectively studied. Diagnostic evaluation of PET/MR images was performed by 2 nuclear medicine physicians. The Kappa consistency test was used to evaluate the diagnostic consistency between PET/MR staging and gold standard in breast cancer. Diagnostic tests were used to evaluate the application value of PET/MR for recurrence detecting. Statistical software SPSS 22.0 was used for the analysis. Results: The diagnostic consistency of PET/MR staging and gold standard was satisfactory(Kappa =0.892). The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of PET/MR for recurrence detecting were respectively 100%, 92.3%, 94.4%, 80%, 100%. Conclusion:^(18)F-FDG PET/MR is highly valuable for staging and recurrence detecting of breast cancer. It is expected to be an accurate and noninvasive method of evaluate breast cancer.
