不同年龄段儿童传染性单核细胞增多症实验室检查分析

Analysis of laboratory examination of infectious mononucleosis in children of different ages

目的分析不同年龄段儿童传染性单核细胞增多症的临床表现及实验室检查数据,为本地区传染性单核细胞增多症的临床诊治提供参考。方法将延安大学附属医院2015年3月—2020年5月收治的141例传染性单核细胞增多症患儿根据年龄分为婴幼儿、学龄前及学龄期3组,回顾性分析各组患儿的临床表现及白细胞、外周血异型淋巴细胞计数等实验室检查项目。结果不同年龄组患儿发热、皮疹、眼睑颜面浮肿、白细胞计数、外周血异型淋巴细胞百分比及肌酸激酶同工酶间的差异均无统计学意义(P>0.05);年龄越大,患儿的浅表淋巴结及肝脾肿大出现率、丙氨酸氨基转移酶异常率越高,差异有统计学意义(P<0.05)。外周血异型淋巴细胞百分比与EB病毒抗体和DNA载量阳性率间的差异无统计学意义(P>0.05),EB病毒DNA载量阳性率比抗体高,差异有统计学意义(P<0.05)。结论各年龄段传染性单核细胞增多症患儿的临床表现及实验室检查存在差异,EB病毒DAN载量比抗体、外周血异型淋巴细胞阳性率高。

Objective To analyze the clinical manifestations and laboratory examinations data of infectious mononu-cleosis in children of different ages,and to provide reference for the clinical diagnosis and treatment of in Yan’an.Methods A total of 141 children with infectious mononucleosis admitted to the Affiliated Hospital of Yan’an University from March 2015 to May 2020 were divided into infants,preschool and school age groups according to their ages.The clinical manifestation and laboratory examination such as the count of white blood cell,peripheral blood atypical lymphocytes were analyzed.Results There were no significant differences in fever,rash,eyelid facial edema,white blood cell count,percentage of peripheral blood atypical lymphocytes,and creatine kinase isoenzymes in different age groups(P>0.05).The older the children were,the higher the occurrence rate of superficial lymph nodes,hepatosplenomegaly and abnormal rate of alanine aminotransferase were found,and the difference was statistically significant(P<0.05).There was no significant difference between the percentage of peripheral blood atypical lymphocytes and the positive rate of EB Virus antibody and DNA load(P>0.05).The positive rate of EBV-DNA load was higher than that of EBV antibody,and the difference was statistically significant(P<0.05).Conclusion There were differences in clinical manifestations and laboratory tests among children with infectious mononucleosis in different age groups,and the positive rate of EBV-DNA was higher than that of EBV antibody and peripheral blood lymphocytes.
