下肢孤立性远端深静脉血栓超声检查实践:上海专家建议

Ultrasonic examination practice of isolated distal deep vein thrombosis:Shanghai expert recommendations

由于孤立性远端深静脉血栓(IDDVT)存在向近心端蔓延和发展为肺动脉栓塞的风险,因此受到越来越多临床医生的关注。但由于IDDVT临床症状较隐匿、下肢静脉超声检查未规范扫查小腿深静脉等原因,导致目前IDDVT检出率较低。为了规范小腿深静脉的超声检查方法,提高IDDVT超声诊断准确率,上海市肺栓塞与深静脉血栓防治联盟参照2009年中国医师协会超声医师分会制定的《血管超声检查指南》及2020年中国超声医学工程学会浅表器官及外周血管超声专业委员会、国家卫健委脑卒中防治工程专家委员会血管超声专业委员会、中国超声医学工程学会颅脑及颈部血管超声专业委员会制定的《腹部及外周静脉血管超声若干临床常见问题专家共识》,并经过深入讨论制定了该实践。

Objective More and more clinicians pay attention to isolated distal deep vein thrombosis(IDDVT)because of its risk of spreading to the proximal end and developing into pulmonary embolism.However,the detecting rate of IDDVT is low due to the occult clinical symptoms of IDDVT and the nonstandard scanning of deep veins in lower limbs of ultrasound doctors.In order to standardize the ultrasonic examination method of deep calf vein and improve the diagnostic accuracy of IDDVT,Shanghai Venous Thromboembolism Alliance(SVTA)developed the ultrasonic examination practice of IDDVT with reference to the"Guidelines for vascular ultrasound examination"formulated by Ultrasound Physicians Branch of Chinese Medical Doctor Association in 2009,"Expert consensus on some clinical problems of abdominal and peripheral venous ultrasonography"formulated by the Professional Committee of Superficial Organ and Peripheral Vascular Ultrasound of the Chinese Medical Ultrasound Engineering,the Professional Committee of Vascular Ultrasound of Stroke Prevention and Treatment Expert Committee of the National Health Commission,Professional Committee of Craniocerebral and Cervical Vascular Ultrasound of the Chinese Medical Ultrasound Engineering in 2020.
