对轻中度呼吸衰竭伴张口呼吸患者进行经面罩与经鼻高流量湿化氧疗的效果对比

Comparison of the effects of mask and nasal high-flow humidified oxygent herapy on patients with mild to moderate respiratory failure with mouth opening

目的:比较对轻中度呼吸衰竭伴张口呼吸患者进行经面罩高流量湿化氧疗与经鼻高流量湿化氧疗的效果。方法:选择120例轻中度呼吸衰竭伴张口呼吸患者作为研究对象。随机将其分为对照组和观察组。对观察组患者进行经面罩高流量湿化氧疗,对对照组患者进行经鼻高流量湿化氧疗,然后比较两组患者的各项临床指标。结果:观察组患者对通气治疗耐受度的评分高于对照组患者,其住院的时间短于对照组患者,其气管插管率低于对照组患者,P<0.05。治疗后,观察组患者的动脉血氧分压(PaO_(2))和血氧饱和度(SaO_(2))均高于对照组患者,其动脉血二氧化碳分压(PCO_(2))低于对照组患者,P<0.05。结论:与对轻中度呼吸衰竭伴张口呼吸患者进行经鼻高流量湿化氧疗相比,对其进行经面罩高流量湿化氧疗的效果更好,能更有效地改善其血气分析指标,提高其对通气治疗的耐受度,缩短其住院的时间,降低其气管插管率。

Objective:To compare the effect of mask high-flow humidified oxygen therapy and nasal high-flow humidified oxygen therapy on patients with mild to moderate respiratory failure accompanied by open-mouth breathing.Methods:120 patients with mild to moderate respiratory failure with open-mouth breathing were selected as subjects.They were randomly divided into control group and observation group.Patients in the observation group were treated with high-flow humidified oxygen therapy via mask,and patients in the control group were treated with high-flow humidified oxygen therapy via nose,and then the clinical indicators of the two groups were compared.Results:The tolerance score of ventilation treatment in observation group was higher than that in control group,the hospitalization time was shorter than that in control group,and the rate of endotracheal intubation was lower than that in control group,P<0.05.After treatment,the arterial partial pressure of oxygen(PaO_(2))and oxygen saturation(SaO_(2))in observation group were higher than those in control group,while the arterial partial pressure of carbon dioxide(PCO_(2))was lower than that in control group,P<0.05.Conclusion:Compared with the nasal high-flow humidified oxygen therapy for patients with mild to moderate respiratory failure with open-mouth breathing,the effect of high-flow humidified oxygen therapy through mask is better,and it can improve the blood gas analysis index more effectively,improve the tolerance to ventilation treatment,shorten the hospitalization time,and re duce the rate of tracheal intubation.
