非酒精性脂肪肝病与老年2型糖尿病性微血管病变相关性研究

Correlation between non-alcoholic fatty liver disease and microvascular complications in type 2 diabetic patients in the elderly

目的探讨非酒精性脂肪肝病与老年2型糖尿病性微血管病变的相关性。方法共纳入226例老年2型糖尿病(T2DM)患者,根据是否合并非酒精性脂肪肝病(NAFLD)分为T2DM合并NAFLD组和单纯T2DM组,并收集其一般资料、相关血液学指标及并发症情况,通过单因素分析和多因素Logistic回归分析NAFLD与2型糖尿病性微血管病变的相关性。结果与单纯T2DM组相比,T2DM合并NAFLD组更高龄,皮下脂肪面积及内脏脂肪面积相对更多,相对更高的腰臀比、BMI,更高的HbA1c、ALT、GGT及TG,同时发现,高血压病、CVD及糖尿病微血管病变的发生率更高(P<0.05)。将单因素中有意义的指标放入多因素Logistic分析模型中发现:糖尿病微血管病变、皮下脂肪面积、HbA1c、TG、CVD有统计学意义(P<0.05)。结论NAFLD与老年2型糖尿病性微血管病变具有相关性。

Objective This study aimed to assess the association between non-alcoholic fatty liver disease(NAFLD)and microvascular complications in type 2 diabetic patients(T2DM)in the elderly.Methods A total of 226 elderly patients with type 2 diabetes were included.According to whether NAFLD is combined,the patients were divided into T2DM with NAFLD group and non-NAFLD group.Collect general conditions,relevant hematology indicators and complications,assess the association between NAFLD and microvascular complications in T2DM by univariate analysis and multivariate logistic regression.Results Compared with the non-NAFLD group,the T2DM with NAFLD group is older,the area of subcutaneous fat and visceral fat is relatively more,and the larger waist to hip ratio,BMI,higher HbA1c,ALT,GGT and TG.In addition,compared with the non-NAFLD group,the T2DM with NAFLD group had a higher presence of hypertension,Cardiovascular disease,and microvascular complications(P<0.05).In the multivariate logistic regression analysis,the results showed that NAFLD was associated with an increased risk of microvascular complications,subcutaneous fat area,HbA1c,TG,CVD in T2DM(P<0.05).Conclusion The NAFLD is associated with microvascular disease in elderly T2DM.
