多指标联合检测在非酒精性脂肪肝诊断中的价值

The value of combined detection of waist circumference,BMI,blood lipid,blood glucose and liver function in the diagnosis of non-alcoholic fatty liver

目的探讨腰围、体质指数、血脂、血糖及肝功能联合检测在非酒精性脂肪肝(NAFLD)诊断及病情评估中的应用价值。方法选取2020年8月到2021年1月到我院健康体检的成年人480例,根据B超诊断脂肪肝并将其分为无、轻度、中度及重度脂肪肝组,并根据腰围、BMI及血脂、血糖、肝功能水平不同各自分组进行统计学分析。结果①不同血脂及FBG水平脂肪肝严重程度存在差异,血脂及FBG异常的患者脂肪肝严重程度高于血脂及FBG水平正常的患者,且脂肪肝的严重程度随着TG、LDL-C、FBG的升高而加重。②腹型肥胖组脂肪肝严重程度明显高于腰围正常组患者。③BMI属超重和肥胖组脂肪肝严重程度高于BMI正常组,BMI属肥胖组脂肪肝严重程度高于超重组。④中重度脂肪肝组肝酶升高程度较轻度脂肪肝组及无脂肪肝组高,轻度脂肪肝组肝酶升高程度较无脂肪肝组高。结论综合评价BMI、腰围、血糖、血脂、肝功能及肝脏超声检查指标对早期发现非酒精性脂肪肝患者具有重要价值。

Objective To explore the application value of combined detection of waist circumference,body mass index,blood lipid,blood glucose level and liver function in the diagnosis and evaluation of non-alcoholic fatty liver(NAFLD).Methods Select 480 adults who went to our hospital for physical examination from August 2020 to January 2021.Fatty liver was diagnosed according to B-ultrasound and divided into none,mild,moderate and severe fatty liver groups,and statistical analysis was performed according to different waist circumference,BMI,blood lipid,blood glucose,and liver function levels.Results①There are differences in the severity of fatty liver with different blood lipids and FBG levels.The severity of fatty liver in patients with abnormal blood lipids and FBG is higher than that in patients with normal blood lipids and FBG levels,and the severity of fatty liver increases with the increasing of TG,LDL-C,FBG.②The severity of fatty liver in the abdominal obesity group was significantly higher than that in the normal waist group.③The severity of fatty liver in the overweight and obesity group was higher than that of the normal BMI group,and the severity of fatty liver in the obese group was higher than that of the overweight group.④The elevation of liver enzymes in the moderate to severe fatty liver group was higher than that in the mild fatty liver group and the non-fatty liver group,and the elevation of liver enzymes in the mild fatty liver group was higher than that of the non-fatty liver group.Conclusion Comprehensive evaluation of BMI,waist circumference,blood glucose,blood lipids,liver function and liver ultrasound examination indicators is of great value for early detection of NAFLD.
