针灸治疗糖尿病胃轻瘫选穴规律和刺灸方法探讨

Analysis on Acupoint Selection and Methods Rules for Diabetic Gastroparesis

目的:运用数据挖掘技术探讨针灸治疗糖尿病胃轻瘫(DGP)的选穴规律和刺灸方法。方法:检索2011年至2020年国家知识基础设施数据库(CNKI)、中国学术期刊数据库(CSPD)、中文科技期刊数据库(CCD)中针灸治疗DGP的临床文献,对最终纳入的73篇文献提取腧穴处方及刺灸方法,用Excel 2010、SPSS Modeler 18.0软件进行统计分析和关联分析。结果:针灸治疗DGP使用频次高的腧穴依次是足三里、中脘、胃俞、内关;胃经、任脉、膀胱经是最常用的经脉;特定穴主要选取交会穴、五输穴、下合穴;腧穴分布以四肢和胸腹为主;关联规则提示中脘-足三里关联程度最高,针灸治疗DGP核心腧穴处方为中脘、足三里、内关、胃俞;毫针刺法治疗DGP最常见。结论:针灸治疗DGP选穴及刺灸方法具有规律性,为临床提供参考。

Objective:The data mining technology was used to explore the rule of acupoint selection and methods of acupuncture and moxibustion in treating diabetic gastroparesis.Methods:The clinical literature of acupuncture for DGP published from 2011 to 2020 was searched in Chinese Journal full-text database(CNKI),Wanfang and VIP databases.A total of 73 literature were selected and acupoint prescriptions and methods were extracted.The software of Excel 2010 and SPSS Modeler 18.0 were used for Statistical analysis and association analysis.Results:the top acupoints used for DGP were Zusanli(ST36),Zhongwan(CV12),Weishu(BL21),Neiguan(PC6);the mostly commonly used meridians were Stomach Meridian of Foot-Yangming、Ren Channel、Bladder Meridian of Foot-Taiyang;the major specific acupoints were the crossing points、five-shu points、xia-he points;the acupoints were mainly distributed in limbs and chest-abdomen;association rules suggest that Zhongwan(CV12)-Zusanli(ST36)has the highest degree of association;the core prescription for DGP was Zhongwan(CV12)、Zusanli(ST36),Neiguan(PC6),Weishu(BL21);filiform needle puncture was the most common treatment for DGP.Conclusion:There is some regularities of acupoints selection and methods related to DGP treated by acupuncture and moxibustion,which provides reference in clinical practice.
