益胃化浊解毒方联合维酶素片治疗慢性萎缩性胃炎胃癌前病变疗效及对Ki-67蛋白表达的影响

Curative effect of Yiwei Huazhuo Jiedu Decoction and vitacoenzyme tablets on precancerous lesions of chronic atrophic gastritis and its effect on the expression of Ki-67 protein

目的探讨益胃化浊解毒方联合维酶素片治疗慢性萎缩性胃炎胃癌前病变疗效及对Ki-67蛋白表达的影响。方法将2017年2月—2018年3月河北省中医院诊治的102例慢性萎缩性胃炎胃癌前病变患者随机分为观察组和对照组,每组51例。对照组采用维酶素片治疗,观察组在对照组治疗基础上加用益胃化浊解毒方治疗,2组均连续治疗3个月。观察2组治疗前后消化不良症状评分、病理组织学变量积分、胃黏膜Ki-67蛋白表达阳性率及SF-36生活质量评分,比较2组患者临床疗效。结果治疗后,2组患者消化不良症状评分、病理组织学变量积分、胃黏膜Ki-67蛋白表达阳性率均明显降低(P均<0.05),SF-36生活质量评分均明显升高(P均<0.05),且观察组上述各指标改善情况均明显优于对照组(P<0.05)。观察组的总有效率为94.1%(48/51),对照组为80.4%(41/51),观察组明显高于对照组(P<0.05)。结论益胃化浊解毒方联合维酶素片能有效改善慢性萎缩性胃炎胃癌前病变患者临床症状及病理组织学病变状态,降低胃黏膜Ki-67蛋白表达阳性率,提高患者生活质量。

Objective It is to explore the curative effect of Yiwei Huazhuo Jiedu Decoction combined with vitacoenzyme tablets on the precancerous lesion of chronic atrophic gastritis and its effect on the expression of Ki-67 protein.Methods A total of 102 patients with chronic atrophic gastritis with gastric precancerous lesions diagnosed and treated in Hebei Provincial Hospital of Traditional Chinese Medicine from February 2017 to March 2018 were randomly divided into an observation group and a control group,with 51 cases in each group.The control group was treated with vitacoenzyme tablets,and the observation group was treated with Yiwei Huazhuo Jiedu Decoction on the basis of the treatment of the control group.Both groups were treated for 3 consecutive months.The scores of dyspepsia symptoms,histopathological variable scores,the positive rate of gastric mucosal Ki-67 protein expression and SF-36 scores of quality of life were observed before and after treatment in the two groups,and the clinical efficacy of the two groups were compared.Results After treatment,the scores of dyspepsia symptoms,histopathological variable scores,and the positive rate of gastric mucosal Ki-67 protein expression in the two groups were significantly reduced(all P<0.05),and the SF-36 scores of quality of life were significantly increased(all P<0.05),and the improvement of the above indicators in the observation group was significantly better than that of the control group(P<0.05).The total effective rate of the observation group was 94.1%(48/51),and that of the control group was 80.4%(41/51),the observation group was significantly higher than the control group(P<0.05).Conclusion Yiwei Huazhuo Jiedu Decoction combined with vitacoenzyme tablets can effectively improve the clinical symptoms and histopathological lesions,reduce the positive rate of Ki-67 protein expression in gastric mucosa,and improve the quality of life of patients with gastric precancerous lesions of chronic atrophic gastritis.
