微小RNA-10a联合甲胎蛋白、糖类抗原50、糖类抗原19-9检测在结直肠腺癌早期诊断中的价值

Value of microRNA-10a combined with AFP,CA_(50) and CA_(199) in early diagnosis of colorectal adenocarcinoma

目的:探究微小RNA-10a(microRNA-10a)联合甲胎蛋白(AFP)、糖类抗原50(CA_(50))、糖类抗原19-9(CA_(199))检测在结直肠腺癌早期诊断中的价值。方法:回顾性分析108例早期结直肠腺癌患者临床资料(结直肠腺癌组),并经倾向性得分匹配法收集与结直肠腺癌组1∶1匹配的108例结直肠良性病变者临床资料(良性组)。比较两组患者一般资料,血清microRNA-10a、AFP、CA_(50)、CA_(199)水平差异,并使用受试者工作特征(ROC)曲线评估血清microRNA-10a、AFP、CA_(50)、CA_(199)及其联合检测对早期结直肠腺癌的诊断价值。结果:两组患者性别、年龄、病变部位比较,差异无统计学意义(均P>0.05)。结直肠腺癌组患者血清microRNA-10a、AFP、CA_(50)、CA_(199)水平显著高于良性组(均P<0.05)。经ROC曲线分析,血清microRNA-10a、AFP、CA_(50)、CA_(199)对早期结直肠腺癌均有较高诊断价值,且四项联合检测诊断价值更高。结论:血清microRNA-10a联合AFP、CA_(50)、CA_(199)检测对早期结直肠腺癌诊断价值较高,可应用于结直肠腺癌筛查。

Objective:To explore the value of microRNA-10a combined with alpha-fetoprotein(AFP),carbohydrate antigen 50(CA_(50))and carbohydrate antigen 19-9(CA_(199))in the early diagnosis of colorectal adenocarcinoma.Methods:The clinical data of 108 patients with early colorectal adenocarcinoma(colorectal adenocarcinoma group)were retrospectively analyzed,and the clinical data of 108 patients with benign colorectal lesions(benign group)matching with colorectal adenocarcinoma group at 1∶1 were collected by propensity score matching method.The general data and the levels of serum microRNA-10a,AFP,CA_(50) and CA_(199) were compared between the two groups,and the receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of serum microRNA-10a,AFP,CA_(50) and CA_(199) and their combined detection in early colorectal adenocarcinoma.Results:There were no statistically significant differences in terms of gender,age and lesion location between the two groups(all P>0.05).The levels of serum microRNA-10a,AFP,CA_(50) and CA_(199) in colorectal adenocarcinoma group were significantly higher than those in benign group(all P<0.05).According to ROC curve analysis,serum microRNA-10a,AFP,CA_(50) and CA_(199) had high diagnostic value on early colorectal adenocarcinoma,and the combined detection of the four had the higher diagnostic value.Conclusion:Serum microRNA-10a combined with AFP,CA_(50) and CA_(199) has higher diagnostic value in the diagnosis of early colorectal adenocarcinoma,and can be used for colorectal adenocarcinoma screening.
