耳内镜下耳屏软骨-软骨膜内植法鼓膜成形术的临床疗效

Clinical Effect of Tympanoplasty With Tragus Cartilage and Perichondrial Implant Under Otoscope

目的探讨经耳内镜下耳屏软骨-软骨膜内植法鼓膜成形术的临床疗效。方法选取2019年6月—2020年9月在科室住院并行耳内镜手术的患者,所有患者均在全身麻醉下进行耳内镜下耳屏软骨-软骨膜内植法鼓膜成形术。术后1个月观察鼓膜生长愈合情况,术后6个月查纯音测听检测听力恢复情况。结果共30例(30耳)成人患者参与此次研究。所选病例均为确诊慢性化脓性中耳炎伴鼓膜穿孔且干耳1个月以上的患者。28例(28耳)穿孔鼓膜经修补成功愈合,愈合率93.3%。术后6个月气导听阈及气骨导差与术前相比明显提高,差异有统计学意义(P<0.05),听力改善明显。结论耳内镜下耳屏软骨-软骨膜内植法鼓膜成形术,成功率高、操作简单、易于开展、并发症少。

Objective To explore the clinical effect of tympanoplasty with tragus cartilage and perichondrial implant under otoscope.Methods Selected patients who were hospitalized in our department from June 2020 to September 2021 and undergo otoscope surgery.All patients underwent tympanoplasty with tragus cartilage and perichondrial implant under otoscope under general anesthesia.The growth of the tympanic membrane was observed one month after the operation,and the hearing recovery was examined six months after the operation.Results A total of 30 adult patients(30 ears)participated in this study.The selected cases were all patients with confirmed chronic suppurative otitis media with perforation of tympanic membrane and dry ears for more than 1 month.28 cases(28 ears)repaired the perforated tympanic membrane and healed successfully,with a healing rate of 93.3%.The air conduction hearing threshold and airbone conduction difference were significantly improved six months after the operation compared with those before the operation,the difference was statistically significant(P<0.05),and the hearing was improved significantly.Conclusion The tympanoplasty with tragus cartilage and perichondrial implant under otoscope has high success rate,simple operation,easy development and few complications.
