摘要
目的观察circRNA MICRA在急性心肌梗死(AMI)患者外周血中水平变化,及其对AMI患者心功能的影响。方法选取2015年9月至2017年11月于江苏省人民医院浦口分院心内科住院就诊的AMI患者共85例,入院即刻均抽取患者外周血约5 mL用于检测MICRA水平,行超声心动图检查记录左心室射血分数(LVEF),其中LVEF≥40%为高LVEF组(44例),其余为低LVEF组(41例),比较两组患者临床资料,分析MICRA与AMI患者心功能的关系。结果(1)低LVEF组肌钙蛋白I(cTnI)、肌酸激酶同工酶(CK-MB)、N末端脑利钠肽前体(NT-proBNP)水平显著高于高LVEF组,而MICRA水平显著低于高LVEF组(t=3.212、3.180、7.990、8.393,P <0.01);(2)ROC曲线评估MICRA对于左心室功能障碍具有一定预测价值,其曲线下面积(AUC)为[0.722(0.647~0.779),P <0.05];(3)MICRA与左心室功能障碍密切相关,其优势比[OR(95%CI)=3.364(2.162~5.235),P <0.05]。结论 MICRA可能是影响AMI患者左心室功能障碍的因素。
Objective To investigate the changes of circRNA MICRA in peripheral blood of patients with acute myocardial infarction(AMI) and the effect of MICRA on cardiac function. Methods 85 AMI patients admitted to the department of cardiology in Pukou Branch of Jiangsu Provincial People’s Hospital from September 2015 to November 2017 were collected, and 5 mL of peripheral blood were obtained from all of them when admitted to hospital immediately for detecting MICRA. Left ventricular ejection fraction(LVEF) was recorded by echocardiography. Patients were divided into high LVEF group(LVEF ≥ 40%, n=44) and low LVEF group(n=41). The clinical parameters between two groups were compared and the relationship of MICRA and cardiac function in patients with AMI was analyzed. Results(1) The levels of cardiac troponin I(cTnI), creatine kinase-MB(CK-MB) and N-terminal pro brain natriuretic peptide(NT-proBNP) in the low LVEF group were significantly higher than those in high LVEF group, while the level of circRNA MICRA was significantly lower than that in high LVEF group(t=2.592, 3.180, 7.990, 8.393;P < 0.01);(2)Receiver operating characteristic(ROC) curve suggested that MICRA had predictive value for left ventricular dysfunction(LVD), and the area under the curve(AUC) was 0.722(0.647-0.779)(P < 0.05);(3) MICRA was closely related to LVD, and odds ratio(OR)(95%CI) was 3.364(2.162-5.235)(P < 0.05). Conclusion MICRA was a factor affecting left ventricular dysfunction in patients with AMI.
