血清淀粉样蛋白A和C-反应蛋白联合检测在小儿感染性疾病早期鉴别中的临床应用价值 被引量:6

摘要 目的研究小儿感染性疾病早期鉴别中血清C-反应蛋白和淀粉样蛋白A联合检测的临床应用价值。方法收集2019年6月至2020年8月本院儿科住院部住院的小儿感染性疾病患儿100例作为研究对象,根据感染病原体培养检测结果分为细菌感染组和病毒感染组,每组各50例。同时收集同期体检30名健康体检儿童作为健康对照组。统计分析血清SAA、CRP水平,并统计分析SAA、CRP阳性情况,分析SAA、CRP单独与联合检测的诊断效能。结果细菌感染组患儿的血清SAA水平显著高于病毒感染组、健康对照组,差异有统计学意义(P<0.05),病毒感染组患儿的血清SAA水平显著高于健康对照组,差异有统计学意义(P<0.05);细菌感染组患儿的血清CRP水平显著高于病毒感染组、健康对照组,差异有统计学意义(P<0.05)。细菌感染组、病毒感染组患儿的SAA阳性率均显著高于健康对照组,差异有统计学意义(P<0.05);细菌感染组患儿的CRP阳性率显著高于病毒感染组、健康对照组,差异有统计学意义(P<0.05)。SAA+CRP联合检测的诊断敏感度、特异度均显著高于SAA、CRP单独检测,差异有统计学意义(P<0.05)。结论小儿感染性疾病早期鉴别中血清C-反应蛋白和淀粉样蛋白A联合检测的临床应用价值高。 Objective To study the clinical application value of combined detection of serum C-reactive protein and amyloid A in the early identification of pediatric infectious diseases.Methods A total of 100 children with infectious diseases in the pediatric inpatient department of our hospital from June 2019 to August 2020 were collected as the research objects.According to the infection pathogen culture test results,they were divided into the bacterial infection group and the virus infection group,with 50 cases in each group.At the same time,30 healthy children under physical examination during the same period were collected as the healthy control group.The serum levels and positive situation of SAA and CRP,the diagnostic efficacy of SAA,CRP and combined detection were statistically analyzed.Results The serum SAA level of children in the bacterial infection group was significantly higher than that of the virus infection group and healthy control group,the difference was statistically significant(P<0.05).The serum SAA level of the virus infection group was significantly higher than that of the healthy control group,the difference was statistically significant(P<0.05).The serum CRP levels of children in the bacterial infection group were significantly higher than those in the virus infection group and the healthy control group,the difference was statistically significant(P<0.05).The SAA positive rate of children in the bacterial infection group and virus infection group was significantly higher than that of the healthy control group,the difference was statistically significant(P<0.05).The CRP positive rate of children in the bacterial infection group was significantly higher than that of the virus infection group and healthy control group,the difference was statistically significant(P<0.05).The diagnostic sensitivity and specificity of SAA+CRP combined detection were significantly higher than SAA or CRP detection,the difference was statistically significant(P<0.05).Conclusion The combined detection of serum C-reactive protein and amyloid A has a high clinical application value in the early identification of pediatric infectious diseases.
