桂枝茯苓方治疗慢性精囊炎瘀血阻滞证的临床疗效及对精液参数、精浆果糖、尿流率的影响

Clinical efficacy of Guizhi Fuling Formula for chronic seminal vesiculitis of blood stasis syndrome and its effect on semen parameters and seminal fructose and urine flow rate

目的观察桂枝茯苓方治疗慢性精囊炎瘀血阻滞证的临床疗效及对患者精液参数、精浆果糖、尿流率的影响。方法将119例慢性精囊炎瘀血阻滞证患者按照随机数字表法分为2组。治疗组61例(后脱落9例)予桂枝茯苓方治疗,对照组58例(后脱落5例)予云南白药胶囊治疗,2组均治疗30 d。比较2组治疗前后精液参数(精子活动率、前向运动精子百分率)、镜下红细胞数量、精浆果糖含量及尿流率[最大尿流率(MFR)、平均尿流率(AFR)],并统计疗效及不良反应发生情况。结果治疗组总有效率88.46%(46/52),对照组总有效率66.04%(35/53),治疗组疗效优于对照组(P<0.05)。治疗后2组精子活动率、前向运动精子百分率均较本组治疗前升高(P<0.05),且治疗后治疗组均高于对照组(P<0.05)。治疗后2组精液镜下红细胞数量较本组治疗前减少(P<0.05),且治疗后治疗组少于对照组(P<0.05);治疗后治疗组精浆果糖含量较本组治疗前升高(P<0.05),且治疗后治疗组高于对照组(P<0.05)。治疗后治疗组MFR、AFR均较本组治疗前升高(P<0.05),且治疗后治疗组均高于对照组(P<0.05)。2组均无不良事件发生。结论桂枝茯苓方治疗慢性精囊炎瘀血阻滞证疗效确切,可以提高精子质量,其机制可能与促进精路通畅及改善精囊腺体的分泌功能有关。

Objective To observe the clinical efficacy of Guizhi Fuling Formula for chronic seminal vesiculitis of blood stasis syndrome,and its effect on semen parameters,seminal fructose and urine flow rate.Methods Totally 119 patients with chronic seminal vesiculitis(blood stasis syndrome)were randomly divided into two groups,61 cases(9 cases of shedding)in treatment group were treated with Guizhi Fuling Formula,58 cases(5 cases of shedding)in control group with yunnan baiyao capsule.Treatment lasted 30 days,the aim was to compare semen parameters(sperm motility,rapid progression motility),the number of erythrocytes under microscope,the content of seminal fructose,urine flow rate[maximum flow rate(MFR),average flow rate(AFR)];the curative effect and adverse reactions were counted.Results The total effective rates in treatment group and control group were 88.46%(46/52),and 66.04%(35/53),respectively,the curative effect being better in treatment group than control group(P<0.05).After treatment,semen parameters in groups were increased than those before treatment(P<0.05),treatment group was higher than control group(P<0.05);the number of erythrocytes under microscope in groups were decreased(P<0.05),which decreased in treatment group when compared with cortrol group(P<0.05);the content of seminal fructose in groups was increased in comparison with that before treatment,which was higher in treatment group than in control group(P<0.05);urine flow rate in groups was increased(P<0.05),which was higher in treatment group versus control group(P<0.05);and without adverse reactions.Conclusion For patients with chronic seminal vesiculitis(blood stasis syndrome),Guizhi Fuling Formula has a definite effect and can improve sperm quality.Its mechanism may be associated with promotion of smooth flow and improvement of secretory function of seminal vesicle.
