基于全民健康覆盖的俄罗斯基本医疗卫生支出绩效评价

Performance Evaluation of Russia’s Primary Health Expenditure Based on Universal Coverage

该文以全民健康覆盖测量指标框架为基础构建评价医疗卫生支出绩效的指标体系,通过洛伦茨曲线、基尼系数、泰尔指数法、数据包络分析法(DEA)和计量回归模型等方法,从基本医疗卫生支出的公平性、效率性和效益性等角度对俄罗斯基本医疗卫生的财政保障机制进行评价。研究结果显示,俄罗斯基本医疗卫生支出分配较为公平,但政府公共卫生支出综合效率偏低,未能达到最优支出规模,卫生资源的规划和管理水平也有待提高,还应在提高基本医疗卫生服务的公平性和有效性等方面进行进一步的改进与提升。

This paper evaluates health expenditure performance by constructing a system of indices based on a framework of the indices to measure universal health coverage.Russia’s fiscal guarantee mechanisms for primary health care is thus evaluated by the system from the perspectives of equality,efficiency and efficacy.The findings of this study indicate that despite the fact that the distribution of Russia’s primary health expenditure is relatively equitable,optimal scale of expenditure has not yet been attained as demonstrated by the country’s low level of comprehensive efficiency,coupled with insufficient planning and management of health resources.
