青少年正常骨面型上下唇厚度生长变化的CBCT研究

A study of growth changes of upper and lower lip thickness in adoescents with normal skeletal type by CBCT

目的:探讨青少年正常骨面型上下唇厚度生长发育的变化。方法:选择240例(女120例,男120例)正畸治疗前的CBCT图像,年龄8~18岁,平均年龄13.2岁,侧貌均角直面型(22°<FH-MP<32°,1°<ANB<5°),基于颈椎骨龄量化分期法(quantitative cervical vertebra maturation,QCVMⅠ~Ⅳ)分为高峰前期组(QCVMⅠ),高峰期组(QCVMⅡ),高峰后期组(QCVMⅢ&Ⅳ)3组,每组男女各40例。将CBCT图像以Dicom文件格式导入Dolphin软件中,截取正中矢状面图像对上下唇厚度进行相关定点及线距测量。结果:男性和女性除了上唇唇峰厚度SPr-Ls从高峰前期至高峰期无统计学意义(P>0.05),其它上下唇厚度测量项目在不同颈椎骨龄分期比较差异均有统计学意义(P<0.05)。在高峰后期,上下唇厚度男女差异均有统计学意义(P<0.05),而颏前点软组织厚度Pog-Pog’在相同颈椎骨龄分期之间,男女差异均无统计学意义(P>0.05)。结论:正常骨面型青少年高峰前期至高峰后期男女上下唇厚度均有增厚,其中高峰期至高峰后期,男性上下唇增厚比女性更为明显。

Objective:To investigate the growth changes of upper and lower lip thickness in adolescents with normal skeletal type.Methods:Totally 240 patients(120 girls and 120 boys)aged 8~18(mean age 13.2 years)with normal skeletal type(22°<FH-MP<32°,1°<ANB<5°)were selected.The subjects were divided into pre-peak period(QCVMⅠ)group,peak period(QCVMⅡ)group and post-peak period(QCVMⅢ&Ⅳ)group based on quantitative cervical vertebra maturation(QCVMⅠ~Ⅳ),with 40 girls and 40 boys for each group.The CBCT images obtained before orthodontic treatment were imported into Dolphin software in Dicom file format.The measurements of the upper and lower lip thickness were taken on the median sagittal plane image.Results:There were significant differences for the upper and lower lip thickness in different stages of QCVM both in girls and boys,except the SPr-Ls between the QCVMⅠand QCVMⅡstage.There were also significant differences in upper and lower lip thickness between girls and boys in the QCVMⅢ&Ⅳstage.But the Pog-Pog’was no significant difference between girls and boys in the same QCVM stage.Conclusion:The upper and lower lip thickness in adoescents with normal skeletal type increased both in girls and boys in different stages of QCVM through the whole pubertal phase.From QCVMⅡto QCVMⅢ&Ⅳstage,the upper and lower lip thickness in males increased more than females.
