徒手抗阻训练治疗颈型颈椎病临床观察

摘要 目的探讨徒手抗阻训练对颈型颈椎病患者的临床疗效。方法选取上海市宝山区中西医结合医院2019年3月—2019年10月的颈型颈椎病患者60例,随机分为对照组与观察组各30例。对照组采取常规物理治疗,观察组在常规物理治疗的基础上加徒手抗阻训练。评价2组患者干预前后颈椎曲度、关节活动度及颈椎功能障碍指数的变化。结果观察组干预3个月后颈椎生理曲度总有效率达96.67%(29/30),其中显著改变17例,较小改变12例,无效1例,治疗前后生理曲度明显改善(P=0.00),且明显优于对照组;而对照组干预前后颈椎曲度改变不明显(P<0.05)。颈椎屈伸、左右旋转、左右侧屈的活动度方面,观察组改善总有效率达86.67%(26/30),其中有效26例、无效4例;治疗前后活动度值差异有统计学意义(P=0.00),且改善显著优于对照组;而对照组干预前后差异无统计学意义(P>0.05)。观察组和对照组颈椎功能障碍指数干预后均有减小(P=0.00),且观察组的指数分值明显低于对照组(P=0.00)。结论徒手抗阻训练可有效改善颈型颈椎病患者颈生理曲度、关节活动度和功能障碍,提高临床疗效。 Objective To explore the clinical effect of bare-handed resistance training on patients of cervical spondylosis.Methods Sixty patients with cervical spondylosis were selected.They were randomly divided into control group(n=30)and observation group(n=30).Conventional physical therapies were applied to participants in control group,while Bare-handed Resistance Training was added to observation group.Changes of cervical curvature,motion range and Neck Disability Index(NDI)were evaluated before and after intervention within two groups.Results The total effective rate of cervical curvature of 30 patients in the observation group after 3 months of intervention was 96.67%(29/30).Among them,17 cases were significantly changed,12 cases were minor changes,and 1 case was ineffective.It showed significant improvement in physiological curvature(P=0.00),and as compared to the control group.The cervical curvature was not significantly changed before and after intervention in the control group(P<0.05).The total effective rate of motion of cervical flexion and extension,left and right rotation and left and right lateral flexion was 86.67%(26/30).Among them,26 cases were effective and 4 cases were invalid(P=0.00).It was improved comparing to the control group and the difference was statistically significant.No significant improvement was observed in the control group after the intervention(P<0.05).NDI scores decreased after intervention both in the observation group and the control group(P=0.00),and the scores of the observation group were significantly lower than that of the control group(P=0.00).Conclusion Bare-handed resistance training can effectively improve the cervical physiological curvature,range of motion and dysfunction of cervical spondylopathy,and improve the clinical efficacy.
