血清IL-1、KL-6表达与输血相关性急性肺损伤发生的相关性 被引量:2

Correlation between expression of serum IL-1 and KL-6 and occurrence of transfusion related acute lung injury

摘要 目的探讨血清白细胞介素-1(IL-1)、涎液化糖链抗原(KL-6)表达与输血相关性急性肺损伤(TRALI)发生的关系。方法选取林州市中心血库2019年2月至2020年5月收治的84例急性大出血患者作为研究对象,所有患者均进行规范化输血治疗,将输血后发生TRALI者纳入发生组,未发生TRALI者纳入未发生组,比较两组年龄、性别、输血量、出血部位与血清IL-1、KL-6水平。结果纳入的84例患者中,12例发生TRALI,发生率为14.29%(12/84),纳入发生组,其余未发生TRALI的72例患者纳入未发生组。两组年龄、输血量、性别、出血部位比较差异未见统计学意义(P>0.05),两组IL-1、KL-6水平比较差异有统计学意义(P<0.05)。经Logistic回归分析结果显示,血清IL-1、KL-6水平升高是引发TRALI的危险因素(OR>1,P<0.05)。结论血清IL-1、KL-6高表达是引发TRALI的危险因素,联合检测IL-1、KL-6水平有助于预测急性大出血患者发生TRALI的风险,为早期采取预防措施提供指导。 Objective To investigate the relationship between the expression of serum IL-1 and KL-6 and the occurrence of transfusion related acute lung injury(TRALI).Methods Eighty-four patients with acute massive hemorrhage in Central Blood Bank of Linzhou from February 2019 to may 2020 were selected as the research objects.All patients were treated with standardized blood transfusion.The patients with TRALI after blood transfusion were included in the occurrence group,and those without TRALI were included in the non occurrence group.The age,gender,blood transfusion volume,bleeding site and serum IL-1,KL-6 levels in the two groups were recorded.Results Among the 84 patients included,12 cases had TRALI,the incidence rate was 14.29%,and they were included in the occurrence group;other 72 cases without TRALI were included in the non occurrence group.The differencens in age,blood transfusion volume,gender and bleeding site were not significantly different between the two group(P>0.05),but the differencens in the levels of IL-1 and KL-6 were significant between the two group(P<0.05).Logistic regression analysis showed that the increase of serum IL-1 and KL-6 levels was the risk factor for TRALI(OR>1,P<0.05).Conclusions The high expression of serum IL-1 and KL-6 is the risk factors of TRALI.The combined detection of IL-1 and KL-6 levels can help to predict the risk of TRALI in patients with acute massive hemorrhage,and provide guidance for early preventive measures.
