重度牙周炎患者种植修复后激光照射治疗对龈沟液炎症因子及种植体的影响 被引量:4

Effect of Laser Radiation Therapy after Implant Restoration on Inflammatory Factors in Gingival Crevicular Fluid and Implant in Patients with Severe Periodontitis

摘要 目的:探讨种植修复后激光照射治疗对重度牙周炎患者的应用效果。方法:回顾性分析2018年2月-2019年7月于本院接受种植修复治疗的72例重度牙周炎患者的临床资料,依据是否进行激光照射分对照组和观察组,每组36例。对照组接受种植修复后进行根面平整治疗,观察组在对照组基础上进行激光照射治疗。比较两组的龈沟液炎症因子及种植体情况。结果:观察组的种植体留存率高于对照组,差异无统计学意义(P>0.05)。治疗前,两组种植体探诊深度(PD)、改良菌斑指数(mPLI)、牙龈乳头指数(PIS)、改良龈沟出血指数(mSBI)比较,差异均无统计学意义(P>0.05);治疗后3个月,两组PD、mPLI、mSBI及PIS值比较,差异均无统计学意义(P>0.05)。治疗后3个月,观察组种植体周围边缘骨吸收量低于对照组,差异有统计学意义(P<0.05)。治疗前,两组IL-6、IL-8、IL-1β水平比较,差异均无统计学意义(P>0.05);治疗后3个月,两组IL-6、IL-8及IL-1β水平均低于治疗前,且观察组低于对照组,差异均有统计学意义(P<0.05)。结论:重度牙周炎患者种植修复后激光照射治疗后,可有效改善龈沟液炎症因子情况,减少种植体周围边缘骨吸收量,且种植体情况良好。 Objective:To investigate the application effect of laser radiation therapy after implant restoration in patients with severe periodontitis.Method:The clinical data of 72 patients with severe periodontitis who received implant restoration in our hospital from February 2018 to July 2019 were retrospectively analyzed,and they were divided into control group and observation group according to whether laser radiation was given or not,36 patients in each group.The control group received root surface leveling treatment after implant restoration,and the observation group received laser radiation therapy on the basis of the control group.Gingival crevicular fluid inflammatory factors and implants were compared between two groups.Result:The implant retention rate in the observation group was higher than that in the control group,the difference was not statistically significant (P>0.05).Before treatment,there were no significant differences in PD,mPLI,mSBI,PIS between two groups (P>0.05);3 months after treatment,there were no significant differences in PD,mPLI,mSBI and PIS between two groups (P>0.05).3 months after treatment,the amount of bone resorption in the observation group was lower than that in the control group,the difference was statistically significant (P<0.05).Before treatment,there was no significant difference in the levels of IL-6,IL-8 and IL-1β between two groups (P>0.05);3 months after treatment,the levels of IL-6,IL-8 and IL-1β in both groups were lower than those before treatment,and those in the observation group were lower than those in the control group,the differences were statistically significant (P<0.05).Conclusion:Laser radiation therapy after implant restoration in patients with severe periodontitis can improve the inflammatory factors in gingival crevicular fluid,reduce the amount of bone resorption and the situations of implant are good.
