拮抗剂方案IVF获卵6枚并发OHSS1例报道及文献复习

Report and Literature Review of A Case of 6 Follicles Obtained by Antagonist IVF Complicated with OHSS

目的:探讨辅助生殖技术体外受精(in vitro fertilization,IVF)中卵巢过度刺激综合征(ovarian hyper stimulation syndrome,OHSS)发生的潜在性、多样性,为临床工作中OHSS的诊治提供思路和措施。方法:对西南医科大学附属医院辅助生殖技术部1例拮抗剂方案获卵6枚诱发重度OHSS的患者治疗过程进行分析。结果:患者拮抗剂方案移植后诱发重度OHSS,经过预防感染、保胎、输注人血白蛋白(20 g/d)等扩容对症治疗,现NT检查已通过。结论:OHSS的发生是充满复杂性、潜在性的,临床上需尽早观察和预处理,降低患者并发风险,妥善处理并发症。

Objective:To explore the potential and diversity of ovarian hyper stimulation syndrome(OHSS)in in vitro fertilization(IVF)of assisted reproductive technology,and provide ideas for the diagnosis and treatment of OHSS in clinical work.Method:The treatment process of a patient with severe OHSS induced by 6 follicles obtained from antagonist program in department of Assisted Reproductive Technology,Affiliated Hospital of Southwest Medical University was analyzed.Result:The patient’s antagonist regimen induced severe OHSS after transplantation,and the NT test had been passed after dilatational symptomatic treatment such as infection prevention,fetal preservation,infusion of human albumin(20 g/d).Conclusion:The occurrence of OHSS is full of complexity and potential,so early observation and pretreatment are needed in clinical practice to reduce the risk of patients and properly handle complications.
