血液透析滤过联合加味酸枣仁汤治疗维持性血液透析失眠患者的效果

Effect of Hemodialysis Filtration Combined with Modified Suanzaoren Decoction in the Treatment of Insomnia Patients Undergoing Maintenance Hemodialysis

目的:观察血液透析滤过(HDF)联合加味酸枣仁汤治疗维持性血液透析(MHD)失眠患者的效果。方法:选取2021年2-5月在本院血透室行MHD治疗的50例患者作为研究对象,随机分为对照组和研究组,每组25例。两组均给予基础治疗(即控制血压、纠正贫血等)、血液透析(HD)治疗。然后对照组采用HD+血液灌流(HP)治疗,研究组采用HD+HDF+加味酸枣仁汤治疗,两组均治疗12周。比较两组治疗前后匹斯堡睡眠质量指数量表(PSQI)和生存质量指数自评量表(SF-36)评分,检测两组治疗前后甲状旁腺激素(PTH)、白蛋白(ALB)、血红蛋白(Hb)。结果:治疗前,两组PSQI评分、SF-36评分及PTH、ALB、Hb水平比较差异均无统计学意义(P>0.05);治疗后,两组PSQI评分、PTH水平均较治疗前降低(P<0.05),研究组均低于对照组(P<0.05);治疗后,两组SF-36评分、Hb水平均较治疗前升高(P<0.05),研究组均高于对照组(P<0.05);治疗后,两组ALB水平与治疗前比较差异无统计学意义(P>0.05),两组治疗后比较差异无统计学意义(P>0.05)。结论:血液透析滤过联合加味酸枣仁汤可有效提高MHD失眠患者的睡眠质量和生存质量,能更好改善症状,值得临床推广应用。

Objective:To observe the effect of hemodialysis filtration (HDF) combined with Modified Suanzaoren Decoction in the treatment of insomnia patients undergoing maintenance hemodialysis (MHD).Method:Fifty patients who received MHD treatment in hemodialysis room of our hospital from February to May 2021 were selected as the study subjects and randomly divided into control group and study group,with 25 patients in each group.Both groups received basic treatment (blood pressure control,anemia correction,etc) and hemodialysis (HD) treatment.Then the control group was treated with HD+ hemoperfusion (HP),and the study group was treated with HD+HDF+Modified Suanzaoren Decoction,both groups were treated for 12 weeks.The scores of Pittsburgh sleep quality index (PSQI) and quality of life index self-rating scale (SF-36) were compared between the two groups before and after treatment,and parathyroid hormone (PTH),albumin (ALB) and hemoglobin (Hb) were detected between the two groups before and after treatment.Result:Before treatment,there were no significant differences in PSQI score,SF-36 score and PTH,ALB,Hb levels between the two groups (P>0.05).After treatment,the PSQI score and PTH level of the two groups were lower than those before treatment (P<0.05),and which in the study group were lower than those in the control group (P<0.05).After treatment,SF-36 score and Hb level in both groups were higher than those before treatment (P<0.05),and which in the study group were higher than those in the control group (P<0.05).After treatment,there was no significant difference in ALB level between the two groups compared with before treatment (P>0.05),and there was no significant difference between the two groups after treatment (P>0.05).Conclusion:Hemodialysis filtration combined with Modified Suanzaoren Decoction can effectively improve the sleep quality and quality of life of MHD patients with insomnia,and can improve symptoms better,which is worthy of clinical application.
