经皮椎间孔镜技术治疗腰椎间盘突出症的临床价值

Clinical Value of Percutaneous Foraminal Endoscopy in the Treatment of Lumbar Disc Herniation

目的:探讨经皮椎间孔镜技术治疗腰椎间盘突出症的临床价值。方法:选取2019年3月-2021年3月本院收治的92例腰椎间盘突出症患者,按照随机数字表法将其随机分为对照组和研究组,每组46例。对照组采用常规开放椎板间开窗术治疗,研究组采用经皮椎间孔镜技术治疗。比较两组手术指标、功能障碍情况、日常生活能力和疼痛程度。结果:研究组术中出血量少于对照组,切口长度、手术时间、住院时间均短于对照组,差异均有统计学意义(P<0.05)。术前,两组ODI评分、ADL评分比较,差异均无统计学意义(P>0.05);术后3个月,两组ODI评分均低于术前,ADL评分均高于术前,且研究组ODI评分低于对照组,ADL评分高于对照组,差异均有统计学意义(P<0.05)。术前,两组VAS评分比较,差异无统计学意义(P>0.05),术后3 d、术后1个月,两组VAS评分均低于术前,且研究组均低于对照组,差异均有统计学意义(P<0.05)。结论:腰椎间盘突出症患者采取经皮椎间孔镜技术治疗效果较好,能减轻患者的疼痛,可改善日常生活能力和肢体功能,促进患者恢复,应推广应用。

Objective:To explore the clinical value of percutaneous foraminal endoscopy in the treatment of lumbar disc herniation.Method:A total of 92 patients with lumbar disc herniation treated in our hospital from March 2019 to March 2021 were selected,they were randomly divided into control group and study group according to random number table,46 cases in each group.The control group was treated with conventional open laminectomy,while the study group was treated with percutaneous foraminal endoscopy.The surgical indicators,dysfunction,daily living ability and pain degree were compared between two groups.Result:The amount of intraoperative blood loss in the study group was less than that in the control group,and the incision length,surgical time and hospital stay were shorter than those in the control group,the differences were statistically significant(P<0.05).Before surgery,there were no significant differences in ODI scores and ADL scores between two groups(P>0.05);3 months after surgery,ODI scores in both groups were lower than those before surgery,and ADL scores in both groups were higher than those before surgery,ODI scores in the study group was lower than that in the control group,and ADL score in the study group was higher than that in the control group,the differences were statistically significant(P<0.05).Before surgery,there was no statistically significant difference in VAS scores between two groups(P>0.05);3 d and 1 month after surgery,VAS scores in both groups were lower than those before surgery,and which in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The treatment effect of percutaneous foraminal endoscopy for patients with lumbar disc herniation is better,which can reduce the pain of patients,improve the ability of daily living and limb function,and promote the recovery of patients,so it should be popularized and applied.
