桂枝附子汤加薏苡仁汤联合依托考昔治疗风寒湿痹型肩周炎36例

Guizhi Fuzi Decoction and Yiyiren Decoction combined with Etoricoxib in the Treatment of 36 Cases of Frozen Shoulder with Wind-cold-Dampness Syndrome

目的:观察桂枝附子汤加薏苡仁汤联合依托考昔治疗风寒湿痹型肩周炎的临床疗效。方法:选择2019年11月至2020年12月在湖北省中医院骨伤科治疗的风寒湿痹型肩周炎患者72例,随机分为治疗组和对照组,每组各36例,治疗组采用桂枝附子汤加薏苡仁汤联合依托考昔片口服,对照组采用依托考昔片口服,连续治疗4周。根据临床症状的改善评定临床疗效,采用视觉模拟评分量表(VAS)进行疼痛测定,采用肩关节功能Constant-Murley评分评价肩关节功能。结果:总有效率治疗组为91.7%(33/36),对照组为77.8%(28/36),组间差异无统计学意义(P>0.05);治疗后,两组VAS评分均较治疗前降低,肩关节功能Constant-Murley评分均较治疗前增高,且治疗组优于对照组,差异均有统计学意义(P<0.05)。结论:桂枝附子汤加薏苡仁汤联合依托考昔治疗风寒湿痹型肩周炎,对患者疼痛,肩关节功能的改善优于单一使用依托考昔片,且不良事件的发生率更低,该联合疗法值得临床推广。

Objective:Observe the clinical efficacy of Guizhi Fuzi Decoction and Yiyiren Decoction combined with Etoricoxib in the treatment of Frozen Shoulder due to Wind-cold-Dampness Syndrome.Methods:A total of 72 patients with periarthritis caused by Wind-cold-Dampness Syndrome who were treated in the Department of Orthopedics and Traumatology of Hubei Provincial Hospital of Traditional Chinese Medicine from November 2019 to December 2020 were selected and randomly divided into treatment group and control group,with 36 cases in each group,The treatment group was treated with Guizhi Fuzi Decoction and Yiyiren Decoction combined with Etoricoxib tablets orally,and the control group was treated with etoricoxib tablets for 4 consecutive weeks.The clinical efficacy was evaluated based on the improvement of clinical symptoms,the visual analog scale(VAS)was used for pain measurement,and the shoulder joint function Constant-Murley score was used to evaluate shoulder joint function.Results:The total effective rate was 91.7%(33/36)in the treatment group and 77.8%(28/36)in the control group.There was no statistically significant difference between the two groups(P>0.05);After treatment,the VAS scores of the two groups were lower than those before treatment,and the Constant-Murley scores of shoulder joint function were higher than those before treatment.The treatment group was better than the control group,and the difference was statistically significan(t P<0.05).Conclusion:Guizhi Fuzi Decoction and Yiyiren Decoction combined with Etoricoxib in the treatment of Frozen shoulder caused by Wind-cold-Dampness Syndrome,pain,and improvement of shoulder joint function is better than using Etoricoxib alone,Moreover,the incidence of adverse events is lower,and the combination therapy is worthy of clinical promotion.
