简短认知行为疗法对抑郁障碍的疗效观察

目的观察简短认知行为疗法(BCBT)对抑郁障碍的疗效。方法选取2018年3月至2020年6月在杭州市第七人民医院和惠州市第二人民医院门诊就诊的抑郁障碍患者97例为研究对象,采用随机数字表法分为3组,即BCBT组(仅给予BCBT)31例、联合治疗组(给予常规药物治疗和BCBT)33例、药物组(仅给予常规药物治疗)33例。观察并比较3组患者治疗前(基线评估)、治疗后(基线评估后第6周)、随访期间(基线评估后第10周)汉密尔顿抑郁量表(HAMD-24)、抑郁自评量表(SDS)评分。结果各组患者治疗后HAMD-24及SDS评分均有所下降,差异均有统计学意义(均P<0.05)。3组患者治疗前及治疗后HAMD-24、SDS评分比较,差异均无统计学意义(均P>0.05);随访期间HAMD-24、SDS评分比较,差异均有统计学意义(均P<0.05),其中BCBT组<联合治疗组<药物组。结论单一BCBT及BCBT联合药物治疗能改善抑郁障碍患者的抑郁症状,且持久效果优于单一药物治疗。

Objective To assess the efficacy of brief cognitive behavioral therapy(BCBT)for depression.Methods From March 2018 to June 2020,97 outpatients with depressive disorder in Hangzhou Seventh People's Hospital and Huizhou Second People's Hospital were randomly assigned to three groups:33 cases in BCBT group received BCBT alone,33 cases in combined treatment group received BCBT combined with medication,and 33 cases in pharmacotherapy group received medication alone.All patients were followed up for 4 weeks,and the scores of Hamilton depression rating scale-24(HAMD-24)and self-rating depression scale(SDS)were assessed before and after treatment.Results After treatment the HAMD-24 and SDS scores in three groups all significantly decreased(all P<0.05).Before and immediately after treatment,there were no significant differences in HAMD-24 and SDS scores among three groups(all P>0.05).During the follow-up,HAMD-24 and SDS scores showed significant differences among three groups(both P<0.05),the scores of pharmacotherapy group were higher than other two groups,while the scores in combination group were lower than those in BCBT alone group.Conclusion BCBT alone and combined with medication can improve depressive symptoms,and the enduring effects are better than those of pharmacotherapy.
