经阴道外用雌激素治疗围绝经期尿路感染的临床可行性分析

Clinical feasibility analysis of external vaginal estrogen for perimenopausal urinary tract infection

目的分析经阴道外用雌激素治疗围绝经期尿路感染的临床可行性。方法选取本院2018年1月至2019年9月收治的200例围绝经期尿路感染患者为研究对象,以随机数字表法分为对照组及观察组,每组各100例,对照组采取常规治疗,观察组加用经阴道外用雌激素治疗,随访6个月,对两组临床疗效进行对比。结果治疗前两组尿白细胞计数、尿道上皮细胞成熟度、阴道pH值、阴道健康评分、氧化应激指标、炎症指标相比较,差异无统计学意义(P>0.05),治疗后各指标与治疗前相比较,差异有统计学意义(P<0.05),观察组治疗后各指标与对照组相比较,差异有统计学意义(P<0.05);观察组总有效率为92.00%、复发率为5.00%,与对照组的80.00%、15.00%相比较,差异有统计学意义(P<0.05);观察组不良反应发生率为10.00%,与对照组的6.00%相比较,差异无统计学意义(P>0.05)。结论经阴道外用雌激素治疗围绝经期尿路感染可行且效果确切,值得在今后临床工作中应用。

Objective:To analyze the clinical feasibility of external vaginal estrogen treatment of perimenopausal urinary tract infections.Methods:200 cases of perimenopausal urinary tract infection patients who were treated at our hospital from January 2018 to September 2019 were selected as the research object.They were divided into control group and observation group by random number table method,with 100 cases in each group.The control group received routine treatment,and the observation group received routine treatment and transvaginal external estrogen treatment;the two groups were followed up for 6 months to compare the clinical efficacy.Results:There were no significant differences in urine white blood cell count,urinary tract epithelial cell maturity,vaginal pH,vaginal health score,oxidative stress index,and inflammation index before treatment(P>0.05).There were statistically significant differences in all indicators after treatment compared with those before treatment(P<0.05),and in the observation group compared with the control group after treatment,there were statistically significant differences in all indicators(P<0.05);the total effective rate and recurrence rate of the observation group were 92.00%and 5.00%respectively,compared with 80.00%and 15.00%of the control group(P<0.05);the incidence of adverse reactions in the observation group was 10.00%,compared with 6.00%in the control group,the difference was not statistically significant(P>0.05).Conclusion:Transvaginal topical estrogen is feasible and effective in treating perimenopausal urinary tract infections,and it is worthy of utilizing in clinical work in the future.
