摘要
目的探究转化生长因子-β(TGF-β)、骨形态发生蛋白-2(BMP-2)及miR-672-5p水平与激素性股骨头坏死(SANFH)的关系。方法分析2018年6月至2021年1月期间于南方医科大学南方医院行各项检查的77例疑似SANFH受检者临床资料,根据临床诊断结果分为确诊组52例与未确诊组25例,比较两组TGF-β、BMP-2、miR-672-5p表达水平,分析上述指标对SANFH的诊断效能。根据股骨头坏死程度分期将确诊组分为轻中度组32例与重度组20例,比较两组TGF-β、BMP-2、miR-672-5p水平,分析上述指标对股骨头坏死程度的评估效能。结果确诊组TGF-β、BMP-2水低于未确诊组,miR-672-5p水平高于未确诊组,差异有统计学意义(P<0.05)。TGF-β、BMP-2、miR-672-5p诊断SANFH的曲线AUC为0.858、0.918、0.904,诊断效能良好(P<0.05)。重度组TGF-β、BMP-2水平低于轻中度组,miR-672-5p水平高于轻中度组,差异有统计学意义(P<0.05)。TGF-β、BMP-2、miR-672-5p评估股骨头坏死程度的曲线AUC为0.769、0.826、0.775,诊断效能良好(P<0.05)。结论 TGF-β、BMP-2、miR-672-5p表达水平与SANFH患者股骨头坏死程度具有显著相关性,用于SANFH诊断、病情程度评估均可表现出良好效能。
Objective To explore the relationship between the levels of transforming growth factor-β(TGF-β),bone morphogenetic protein-2(BMP-2)and miR-672-5 p and steroid-induced avascular necrosis of the femoral head(SANFH). Methods The clinical data of 77 subjects with suspected SANFH who underwent various examinations in Nanfang Hospital,Southern Medical University from June 2018 to January 2021 were analyzed. According to the clinical diagnostic results,the patients were divided into 52 cases in the confirmed group and 25 cases in the undiagnosed group. The expression levels of TGF-β,BMP-2 and miR-672-5 p were compared between the two groups,and the diagnostic efficiency of the above indicators on SANFH were analyzed. According to the femoral head necrosis degree,the patients in the confirmed group were classified as the mild-to-moderate group(32 cases)and the severe group(20 cases),and the levels of TGF-β,BMP-2 and miR-672-5 p were compared,and the evaluated efficiency of the above indicators on degree of the femoral head necrosis was analyzed. Results The levels of TGF-β and BMP-2 in the confirmed group were lower than those in the undiagnosed group while the level of miR-672-5 p was higher than that in the undiagnosed group(P<0.05). The AUC values under curves of the above indicators in diagnosing SANFH were 0.858,0.918 and0.904,with good diagnostic efficiency(P<0.05). The levels of TGF-β and BMP-2 in the severe group werelower than those in the mild-to-moderate group while the level of mi R-672-5 p was higher than that in the mild-tomoderate group(P<0.05). The curve AUC of TGF-β,BMP-2,mi R-672-5 p to assess the degree of femoral head necrosis was 0.769,0.826,0.775,and the diagnostic efficiency was good(P<0.05). Conclusion The expression levels of TGF-β,BMP-2 and mi R-672-5 p are significantly correlated with the degree of femoral head necrosis in patients with SANFH,and they can be used for SANFH diagnosis and disease evaluation.
