多囊卵巢综合征患者的血清IGF-1、PRL水平与氯米芬胶囊治疗效果的关系分析

Analysis of the relationship between serum IGF-1 and PRL levels in patients with polycystic ovarian syndrome and the therapeutic effect of clomiphene capsules

目的探讨多囊卵巢综合征(PCOS)患者的血清胰岛素样生长因子-1(IGF-1)、催乳素(PRL)水平与氯米芬胶囊治疗效果的关系。方法选取2018年1月至2020年2月我院收治的PCOS患者86例为研究对象,检测患者的血清IGF-1、PRL水平,给予患者氯米芬胶囊治疗4个疗程后对其临床疗效进行评价。根据临床疗效将患者分为治疗有效组和治疗无效组,比较两组患者的血清IGF-1、PRL水平;对影响氯米芬胶囊治疗PCOS患者临床效果的因素进行Logistic回归分析;绘制受试者工作特征曲线(ROC),评估根据血清IGF-1、PRL水平预测氯米芬胶囊治疗PCOS患者临床效果的价值。结果治疗4个疗程后,86例PCOS患者中治疗有效患者57例、治疗无效患者29例。治疗无效组患者的血清IGF-1、PRL水平均显著高于治疗有效组患者,差异有统计学意义(P<0.05)。Logistic回归分析结果显示,血清IGF-1、PRL水平是影响氯米芬胶囊治疗PCOS患者临床效果的独立因素(P<0.05)。根据血清IGF-1、PRL水平预测氯米芬胶囊治疗PCOS患者临床效果的ROC曲线下面积(AUC)分别为0.845、0.752。结论多囊卵巢综合征患者血清IGF-1、PRL水平与氯米芬胶囊治疗效果关系密切,是影响患者临床治疗效果的独立因素,根据其水平预测患者的临床治疗效果具有一定的应用价值。

Objective To investigate the relationship between serum insulin-like growth factor-1(IGF-1)and prolactin(PRL)levels in patients with polycystic ovarian syndrome(PCOS)and the therapeutic effect of clomiphene capsules.Methods A total of 86 patients with PCOS admitted to our hospital from January 2018 to February 2020 were selected as the research objects.The serum IGF-1 and PRL levels of the patients were detected,and the clinical efficacy of giving clomiphene capsules to patients was evaluated after 4 courses of treatment.Patients were divided into effective group and ineffective group according to the clinical efficacy,and serum IGF-1 and PRL levels were compared between the two groups.Logistic regression analysis was conducted on the factors influencing the clinical effect of clomiphene capsules in the treatment of PCOS patients.The receiver operating characteristic(ROC)curve was drawn to evaluate the value of predicating the clinical effect of clomiphene capsules in the treatment of PCOS patients based on the serum IGF-1 and PRL levels.Results After 4 courses of treatment,57 of the 86 PCOS patients were effectively treated and 29 patients were ineffective.The serum IGF-1,PRL levels of patients with ineffective treatment were significantly higher than those of patients with effective treatment,and the differences were statistically significant(P<0.05).The results of logistic regression analysis showed that serum IGF-1,PRL levels were the independent influencing factors for the clinical effect of clomiphene capsules in PCOS patients.According to the serum IGF-1 and PRL levels,the area under the ROC curve(AUC)for predicating the clinical effect of clomiphene capsules in the treatment of PCOS patients was respectively 0.845 and 0.752.Conclusion The serum IGF-1 and PRL levels in PCOS patients are closely related to the therapeutic effect of clomiphene capsules,which are independent factors affecting the clinical therapeutic effect of patients.It has certain application value to predict the clinical therapeutic effect of patients according to the serum IGF-1 and PRL levels.
