桥本氏甲状腺炎患者雌激素与淋巴细胞亚群相关性研究 被引量:1

Correlation between Estrogen and Lymphocyte Subsets in Patients with Hashimoto’s Thyroiditis

摘要 目的探讨桥本氏甲状腺炎(HT)患者体内雌激素对淋巴细胞亚群是否存在影响.方法依据桥本氏甲状腺炎诊断要点,选择42例围绝经期HT女性患者为病例组,38例同期健康女性为对照组,应用化学发光法检测血清中甲状腺素、超敏促甲状腺素、甲状腺过氧化物酶抗体(TPO-Ab)、甲状腺球蛋白抗体(TG-Ab)、雌二醇(E2)等指标的含量,应用流式细胞仪检测外周血T淋巴细胞(总T细胞、辅助性T细胞、细胞毒性T细胞、调节性T细胞等T细胞亚群)、B淋巴细胞和NK细胞等淋巴细胞亚群百分率.结果与对照组比较,病例组TSH、TG-Ab、TPO-Ab和E2明显升高,FT4明显降低,病例组NK细胞和B细胞百分率明显升高,CD4^(+)CD25^(+)T细胞百分率明显降低,差异具有统计学意义(P<0.05或P<0.01).病例组内雌二醇(E2)偏高组B细胞检测值高于雌二醇偏低组,差异具有统计学意义(P<0.01).结论淋巴细胞亚群在HT中发挥了细胞免疫学作用,同时雌激素对淋巴细胞亚群存在一定的影响. Objective Investigate the effect of estrogen on lymphocyte subsets in patients with Hashimoto’s thyroiditis.Method According to the diagnosis points of Hashimoto’s thyroiditis,42 cases of perimenopausal women were selected as the case group,and 38 cases of healthy women in the same period as the control group.The levels of thyroid hormone,TPO-Ab,TG-Ab and E2 in serum were detected by chemiluminescence method.The percentage of T lymphocyte subsets(total T cell,helper T cells,cytotoxic T cells,regulatory T cells),B lymphocytes and NK cells were detected by flow cytometry.Results Compared with the control group,TSH,TG-Ab,TPO-Ab and E2 in the case group were significantly increased,FT4 was significantly decreased,the percentage of NK cells and B cells was significantly increased,and the percentage of CD4^(+)CD25^(+)T cells was significantly decreased(P<0.05 or P<0.01).In the case group,the detection value of B cells in the high estradiol group was higher than that in the low estradiol group,and the difference was statistically significant.Conclusion Lymphocyte subsets play an important role in cell immunology in Hashimoto’s thyroiditis,and estrogen has some effects on lymphocyte subsets.
