摘要
目的比较基于扫频源相干光层析成像术(SS-OCT)原理的新型IOL Master与传统型IOL Master在眼部生物学测量数据的一致性及人工晶状体度数预测的准确性。方法前瞻性临床研究。共纳入在上海交通大学附属第六人民医院拟手术96例老年性白内障患者,所有眼的测量均在暗室里进行。两种仪器分别对每位患者的AL、K1、K2和ACD进行测定,以评价设备间的一致性。此外应用两种仪器内置的SRK-T公式、Hoffer Q公式及Haigis公式比较两种仪器预测人工晶状体度数的精确性。结果本研究最终纳入96例老年性白内障患者,患者年龄为(64.11±14.59)岁(40~90岁),两种仪器测量眼轴长度方面无明显统计学差异(P=0.067)。两种仪器测量前房深度相比相差约0.07 mm,差异有统计学意义(P=0.023)。传统型IOL Master测量得到角膜曲率K1及K2均较IOL Master 700高(均P<0.001),两种仪器使用内置的SRK-T公式、Hoffer Q公式及Haigis公式计算人工晶状体度数结果均存在统计学差异(P=0.011,P<0.001,P<0.001)。结论IOL Master 700与传统型IOL Master在眼轴测量上一致性较好,但在测量前房深度、角膜平坦轴曲率K1及角膜陡峭轴曲率K2值不可相互替代。两种仪器预测人工晶状体度数精确性相似。
Objective To assess the consistency of the new IOL Master based on the SS-OCT principle(IOL Master700)and the traditional IOL Master in ocular biometry measurement and the accuracy in estimating the intraocular lens power.Methods A total of 96 patients who underwent cataract surgery at the Sixth People's Hospital affiliated to the Shanghai Jiaotong University were included in the analysis.All measurements were performed in the darkroom.Both the traditional and new IOL Master were used to measure each patient's AL,K1,K2,and ACD to assess consistency.In addition,the SRK-T formula,Hoffer Q formula and Haigis formula built-in these two instruments were used to compare the accuracy of these two instruments in predicting the IOL power.Results The study included 96 patients with age-related cataract.The mean age of the patients was(64.11±14.59)years old(range:40-90 years).There was no significant difference in the measured axial length between the two instruments(P=0.067).The anterior chamber depth measured by these two instruments differed by about 0.07 mm,and the difference was statistically significant(P=0.023).The corneal curvature K1 and K2 obtained by the traditional IOL Master were higher than the IOL Master700(P<0.001).The calculated IOL power by the SRK-T formula,Hoffer Q formula and Haigis formula were all significantly different between the two instruments(P=0.011,P<0.001,P<0.001).Conclusions The IOL Master700 and the traditional IOL Master have good consistency in axial length measurement,but were not interchangeable in the measurement of corneal curvature(K1,K2)or anterior chamber depth.The two instruments show similar accuracies in IOL power estimation.
