摘要
目的探讨天麻钩藤饮治疗原发性高血压的临床效果。方法选取原发性高血压患者109例,按随机数字表法分为2组:对照组54例,采用常规西药治疗;观察组55例,在对照组治疗基础上加用天麻钩藤饮治疗。比较2组临床疗效、治疗前后血压变异性及血管内皮保护机制[血管紧张素Ⅱ(AngⅡ)、内皮微颗粒(EMPs)及内皮依赖性舒张功能(FMD)水平]。结果观察组总有效率高于对照组(P<0.05);与治疗前比较,2组治疗后24 h、白昼、夜间及晨峰收缩压及舒张压显著降低,且观察组上述各时段收缩压及舒张压降低更为明显(均P<0.05);与治疗前比较,2组治疗后AngⅡ、EMPs水平显著降低,FMD水平显著升高,且观察组治疗后上述指标变化更为明显(均P<0.05)。结论天麻钩藤饮治疗原发性高血压效果确切,能够通过平肝补肾、清热活血,扩张血管,修复内皮损伤细胞等机制改善临床疗效、血压变异性及血管内皮保护机制。
Objective To investigate the clinical effect of Gastrodia and Uncaria decoction on essential hypertension.Methods 109 essential hypertension patients were randomly treated with conventional Western medicine alone(control group,54 patients)or in combination with Gastrodia and Uncaria decoction(observation group,55 patients).Clinical efficacy,blood pressure variability,and levels of angiotensinⅡ(AngⅡ),endothelial microparticles(EMPs)and flow-mediated vasodilation(FDM)were compared between the two groups.Results The total effective rate in the observation group was higher than that in the control group(P<0.05).After treatment,24-hour,day-time,night-time and morning peak systolic blood pressure and diastolic blood pressure decreased in both groups.Furthermore,levels of AngⅡand EMPs were reduced and FMD was enhanced in both groups.Moreover,the changes in these indicators in the observation group were more obvious than those in the control group(P<0.05).Conclusion Gastrodia and Uncaria decoction can effectively improve clinical efficacy and regulate blood pressure variability and vascular endothelial protection mechanism in patients with essential hypertension by calming liver,tonifying kidney,clearing heat,promoting blood circulation,dilating blood vessels,and repairing damaged endothelial cells.
