颈椎骨折脱位合并脊髓损伤76例临床护理体会

Clinical Nursing Experience of 76 Cases of Cervical Fracture and Dislocation Combined with Spinal Cord Injury

目的探究不同护理方案用于颈椎骨折脱位并发脊髓损伤患者的临床护理效果,为临床护师选择合理护理方案提供参考。方法选择我院2017年1月至2019年6月收治的76例颈椎骨折脱位并发脊髓损伤患者为研究对象,依据随机数字表法,将研究对象分成对照组及试验组(每组38例),对照组患者接受常规护理,试验组患者接受综合优质护理干预,对两组患者接受护理前后的脊椎功能以及护理后的疼痛情况以及生活质量等相应指标进行比较分析。结果试验组患者接受护理干预后JOA评分、QOL量表评分均明显高于对照组,VAS分值低于对照组,两组患者的数据进行对比后差异有统计学意义(P<0.05)。干预后两组ASIA分级相比于护理前有所提升,生存质量显著提高;干预后试验组ASIA分级程度优于对照组,差异有统计学意义(P<0.05)。结论在颈椎骨折脱位合并脊髓损伤患者的临床护理方式选择中,综合优质护理干预的效果更好,建议在临床护理工作中推广应用。

Objective To explore the clinical nursing effect of different nursing programs for patients with cervical fracture and dislocation complicated with spinal cord injury,so as to provide reference for clinical nurses to choose reasonable nursing programs.Methods Seventy six patients with cervical spine fracture and dislocation complicated with spinal cord injury admitted to our hospital from January 2017 to June 2019 were selected as the research objects.According to the random number table method,the research objects were randomly divided into the control group and the experimental group(38 cases in each group).The patients in the control group received routine nursing,the patients in the experimental group received comprehensive and high-quality nursing intervention.The spinal function,pain and quality of life of the two groups before and after nursing were compared.Results After nursing intervention,JOA score and QOL score of experimental group was significantly higher than that of control group,VAS score was lower than that of control group,there was significant difference between the two groups(P<0.05).After the intervention,the ASIA classification of the two groups was improved compared with that before the nursing,and the quality of life was significantly improved;after the intervention,the ASIA classification of the experimental group was better than that of the control group,and the difference was statistically significant(P<0.05).Conclusion In the selection of clinical nursing methods for patients with cervical fracture and dislocation combined with spinal cord injury,the effect of comprehensive high-quality nursing intervention is better.It is suggested that it should be popularized and applied in clinical nursing work.
