麦角新碱联合卡贝缩宫素对前置胎盘剖宫产产妇相关指标的影响

Effect of ergometrine combined with carbetocin on related indexes of cesarean section parturients with placenta previa

目的探究麦角新碱联合卡贝缩宫素对前置胎盘剖宫产产妇相关指标的影响。方法选取我院收治的200例前置胎盘剖宫产产妇作为研究对象,将其随机分为观察组(105例,麦角新碱+卡贝缩宫素)和对照组(95例,卡贝缩宫素)。比较两组的出血情况、凝血功能指标、应激反应指标及子宫复旧情况。结果观察组的止血时间、术中出血量、术后2 h出血量、术后24 h出血量、血红蛋白下降水平均显著低于对照组(P<0.05)。产后24 h,两组的APTT、PT均缩短,FIB水平升高,且观察组优于对照组(P<0.05)。治疗后24 h,两组的NO、NE、Ang-Ⅱ水平均降低,且观察组低于对照组(P<0.05)。产后1、3、5 d,观察组的子宫下降幅度均大于对照组(P<0.05)。结论对前置胎盘剖宫产产妇给予麦角新碱联合卡贝缩宫素治疗,可防治产后出血,改善术后凝血功能及应激状况,促进子宫复旧。

Objective To explore the effect of ergometrine combined with carbetocin on related indexes of cesarean section parturients with placenta previa.Methods A total of 200 parturients of cesarean section with placenta previa admitted in our hospital were selected as the research objects and randomly divided into observation group(105 cases,ergometrine+carbetocin)and control group(95 cases,carbetocin).The bleeding,coagulation function indexes,stress response indexes and uterine involution of the two groups were compared.Results The hemostatic time,intraoperative bleeding,blood loss 2 h after surgery,blood loss 24 h after surgery and precipitation under hemoglobin in the observation group were significantly lower than those in the control group(P<0.05).At 24 h postpartum,APTT and PT of the two groups were shortened,and the level of FIB increased,and those in the observation group were better than the control group(P<0.05).At 24 h after treatment,the levels of NO,NE and Ang-Ⅱ in the two groups decreased,and those in the observation group were lower than the control group(P<0.05).At 1,3,and 5 d postpartum,the uterine decline in the observation group was greater than that in the control group(P<0.05).Conclusion The treatment of ergometrine combined with carbetocin for parturients of cesarean section with placenta previa can prevent postpartum hemorrhage,improve postoperative blood coagulation and stress,and promote uterine involution.
