中药坐浴联合改良自动痔疮套扎术在混合痔患者中的应用价值

Application value of traditional Chinese medicine hip bath combined with modified automatic ligation of hemorrhoids in patients with mixed hemorrhoids

目的探讨中药坐浴联合改良自动痔疮套扎术在混合痔患者中的应用价值。方法根据随机数字表法将我院2017年1月至2020年1月收治的60例混合痔患者分为对照组和观察组,各30例。对照组实施改良自动痔疮套扎术治疗,观察组实施改良自动痔疮套扎术+中药坐浴治疗。比较两组患者的炎症因子水平、创面渗出物消失时间、创面愈合时间、住院时间、舒适度、心理状态、睡眠质量及生活质量。结果手术后第5天,两组患者的IL-6、CRP、PCT水平均降低,且观察组低于对照组(P<0.05)。观察组的创面渗出物消失时间、创面愈合时间、住院时间均短于对照组(P<0.05)。治疗后,两组的GCQ评分显著升高,SAS、SDS、PSQI评分显著降低,且观察组优于对照组(P<0.05)。治疗后,两组的各项WHOQOL-BREF评分显著升高,且观察组高于对照组(P<0.05)。结论在改良自动痔疮套扎术后实施中药坐浴可减轻混合痔患者的肛周炎症反应,促使创面尽快愈合,有利于提高患者的舒适度,改善其心理状况、睡眠状况及生活质量。

Objective To explore the application value of traditional Chinese medicine hip bath combined with modified automatic ligation of hemorrhoids in patients with mixed hemorrhoids.Methods Sixty patients with mixed hemorrhoids treated in our hospital from January 2017 to January 2020 were divided into control group and observation group according to the random number table method,with 30 cases in each group.The control group was treated with modified automatic ligation of hemorrhoids,and the observation group was treated with modified automatic ligation of hemorrhoids+traditional Chinese medicine hip bath.The levels of inflammatory factors,disappearance time of wound exudate,wound healing time,hospital stay,comfort,psychological state,sleep quality and quality of life were compared between the two groups.Results On the 5th day after operation,the levels of IL-6,CRP and PCT decreased in both groups,and those in the observation group were lower than the control group(P<0.05).The disappearance time of wound exudate,wound healing time and hospital stay in the observation group were shorter than those in the control group(P<0.05).After treatment,the GCQ score of the two groups increased significantly,and the SAS,SDS and PSQI scores decreased significantly,and those of the observation group were better than the control group(P<0.05).After treatment,the WHOQOL-BREF scores of the two groups increased significantly,and those of the observation group were higher than the control group(P<0.05).Conclusion The implementation of traditional Chinese medicine hip bath after modified automatic ligation of hemorrhoids can reduce the perianal inflammatory reaction of patients with mixed hemorrhoids,promote the wound healing as soon as possible,increase the comfort of patients,and improve their psychological status,sleep status and quality of life.
