周围神经阻滞和局部浸润麻醉在经会阴前列腺活检中的镇痛效果比较

Comparison of analgesic effect between periprostatic nerve block and periprostatic local infiltration anesthesia in transperineal prostate biopsy

目的:比较周围神经阻滞麻醉(PPNB)及周围局部浸润麻醉(PLIA)在经会阴前列腺活检中的镇痛效果。方法:选取2017年6月至2020年11月在包头市中心医院行经会阴前列腺活检的患者83例,随机分为PPNB组43例和PLIA组40例。比较两组穿刺操作时间、术中及术后各时期的VAS得分、取材期每分钟的VAS得分、并发症情况。结果:PPNB组平均穿刺操作时间低于PLIA组(P<0.01);PPNB组活检取材期、术后1 h和2 h、活检取材期每分钟的VAS得分均低于PLIA组(P<0.05);两组并发症发生率比较差异无统计学意义(P>0.05)。结论:与PLIA相比,在经会阴前列腺活检时采用PPNB具有操作时间更短、术中及术后镇痛效果更好等优势,且安全性好,有较高的临床应用价值。

Objective:To compare the analgesic effects between periprostatic nerve block anesthesia(PPNB)and periprostatic local infiltration anesthesia(PLIA)in transperineal prostate biopsy.Methods:83 patients who underwent transperineal prostate biopsy in Baotou Central Hospital from June 2017 to November 2020 were selected and randomly divided into the PPNB group(n=43)and PLIA group(n=40).The puncture operation time,VAS score during operation and each postoperative period,VAS score per minute during the sampling period,and complications were compared between the two groups.Results:The average puncture operation time in the PPNB group was lower than that in the PLIA group(P<0.01);the VAS scores of the biopsy sampling period,1 h and 2 h after operation and each minute during sampling period in the PPNB group were lower than those in the PLJA group(P<0.05);there was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Compared with PLIA,PPNB in transperineal prostate biopsy has the advantages of shorter operation time,better intraoperative and postoperative analgesic effect with high safety,which has high clinical value.
