全血C反应蛋白和白细胞计数在儿科细菌性肺炎中的应用价值 被引量:2

摘要 目的对细菌性肺炎、非细菌性肺炎、健康儿童的白细胞计数(WBC)和全血C反应蛋白(CRP)水平进行检测分析,研究WBC和全血CRP水平联合检验在儿科细菌性肺炎诊断价值应用。方法50例细菌性肺炎患儿作为细菌组,50例非细菌性肺炎患儿作为非细菌组,50例儿童保健科健康儿童作为对照组。三组均采用乳胶免疫比浊法检测全血CRP水平、仪器分析法检测WBC水平。比较三组CRP、WBC水平,细菌组与非细菌组CRP、WBC阳性率。结果细菌组的CRP水平为(33.45±5.50)mg/L、WBC水平为(13.84±4.87)×10^(9)/L,高于非细菌组的(5.91±1.90)mg/L、(6.98±1.80)×10^(9)/L和对照组的(2.29±0.95)mg/L、(6.85±0.54)×10^(9)/L,差异有统计学意义(P<0.05)。细菌组患儿CRP阳性率为88%、WBC阳性率为50%,均高于非细菌组的16%、12%,差异有统计学意义(P<0.05)。结论将全血CRP和WBC联合检测同时结合临床症状可以在儿科细菌性肺炎的早期诊断及鉴别中呈现更高的应用价值。 Objective To study the clinical value of combined detection of white blood cell counts(WBC)and whole blood C-reactive protein(CRP)levels in the early diagnosis of pediatric bacterial pneumonia by detection and analysis of WBC and CRP levels in bacterial pneumonia,non-bacterial pneumonia,and healthy children.Methods 50 children with bacterial pneumonia served as the bacterial group,50 children with non-bacterial pneumonia served as the non-bacterial group,and 50 healthy children from the Child Health Department served as the control group.All three groups were tested for whole blood CRP levels and WBC levels by latex immunoturbidimetric assay and instrumental assay.Comparison was made on CRP and WBC levels of the three groups,and the positive rates of CRP and WBC between the bacterial group and non-bacterial group.Results In the bacterial group,the CRP was(33.45±5.50)mg/L and WBC was(13.84±4.87)×10^(9)/L,which were higher than(5.91±1.90)mg/L and(6.98±1.80)×10^(9)/L in the non-bacterial group,and(2.29±0.95)mg/L and(6.85±0.54)×10^(9)/L in the control group,and the differences were statistically significant(P<0.05).In the bacterial group,the positive rate of CRP was 88%,and the positive rate of WBC was 50%,both were higher than 16%and 12%in the non-bacterial group,and the difference was statistically significant(P<0.05).Conclusion The combined analysis of whole blood CRP and WBC combined with clinical symptoms can show higher application value in the early diagnosis and differential diagnosis of pediatric bacterial pneumonia.
