诱导化疗联合同期调强放化疗与同期调强放化疗联合辅助化疗治疗局部晚期鼻咽癌的疗效观察

Observation on the efficacy of induction chemotherapy combined with concurrent intensity-modulated radiotherapy vs concurrent intensity-modulated radiotherapy combined with adjuvant chemotherapy in the treatment of locally advanced nasopharyngeal carcinoma

目的比较对局部晚期鼻咽癌患者应用诱导化疗联合同期调强放化疗与同期调强放化疗联合辅助化疗治疗的临床疗效。方法65例局部晚期鼻咽癌患者,采用随机数字表法分为对照组(30例)和观察组(35例)。观察组患者接受同期调强放化疗联合辅助化疗治疗,对照组患者接受诱导化疗联合同期调强放化疗。比较两组近期疗效、无远处转移生存率以及5年生存率。结果观察组的治疗总有效率51.43%与对照组的56.67%比较差异无统计学意义(P>0.05)。观察组患者无远处转移生存率77.14%以及5年生存率82.86%与对照组的76.67%、83.33%对比,差异均无统计学意义(P>0.05)。结论局部晚期鼻咽癌患者应用诱导化疗联合同期调强放化疗治疗,其近期疗效略高于同期调强放化疗联合辅助化疗,但其产生的不良反应也要高于同期调强放化疗联合辅助化疗治疗,且两种治疗方式的远期疗效无显著差异。

Objective To compare the clinical efficacy of induction chemotherapy combined with concurrent intensity-modulated radiotherapy vs concurrent intensity-modulated radiotherapy combined with adjuvant chemotherapy in the treatment of locally advanced nasopharyngeal carcinoma.Methods A total of 65 patients with locally advanced nasopharyngeal carcinoma were divided into control group(30 cases)and observation group(35 cases)according to random numerical table.Patients in the observation group received concurrent intensity-modulated radiotherapy combined with adjuvant chemotherapy,and patients in the control group received induction chemotherapy combined with concurrent intensity-modulated radiotherapy.The shortterm efficacy,distant metastasis-free survival rate,and 5-year survival rate were compared between the two groups.Results The total effective rate of the observation group was 51.43%,which was not statistically different from 56.67%in the control group(P>0.05).The observation group had distant metastasis-free survival rate of 77.14%and 5-year survival rate of 82.86%,which was not statistically significant compared with 76.67%and 83.33%of the control group(P>0.05).Conclusion For patients with locally advanced nasopharyngeal carcinoma,the short-term efficacy of induction chemotherapy combined with concurrent intensity-modulated radiotherapy is higher than that of concurrent intensity-modulated radiotherapy combined with adjuvant chemotherapy,but its adverse reactions are also higher than those of concurrent intensity-modulated radiotherapy combined adjuvant chemotherapy,and there is no significant difference in the long-term efficacy of the two treatment methods.
