AFP、AFP-L3%与GP73联合检测对原发性肝癌的诊断价值

The Diagnostic Value of Combined Detection of AFP,AFP-L3%and GP73 for Primary Liver Cancer

目的:探讨甲胎蛋白(AFP)、甲胎蛋白异质体L3比率(AFP-L3%)与高尔基体蛋白73(GP73)联合检测对原发性肝癌(PHC)的诊断价值。方法:选择2019年4月~2021年4月某院收治的86例疑似PHC患者,所有入选者均检测AFP、AFP-L3%、GP73阳性情况。以病理检查结果作为金标准,比较良性病变组与PHC组AFP、AFP-L3%、GP73水平,分析AFP、AFP-L3%、GP73单独与联合检测诊断PHC的应用价值。结果:86例疑似PHC患者经病理检查确诊48例为PHC,占比为55.81%,38例为良性肝脏疾病,占比为44.19%;PHC组AFP、AFP-L3%、GP73水平均高于良性病变组,差异有统计学意义(P<0.05);AFP、AFP-L3%、GP73联合诊断PHC的灵敏度为91.67%(44/48)、特异度92.11%(35/38)、准确度91.86%(79/86)、阳性预测值93.62%(44/47)、阴性预测值89.74%(35/39)均高于AFP、AFP-L3%、GP73各项单独检测,差异有统计学意义(P<0.05)。结论:在PHC诊断中采用AFP、AFP-L3%、GP73联合检测,能够提高诊断准确度、灵敏度,为早期诊断、治疗提供更多参考依据,具有较高的应用价值。

Objective:To explore the diagnostic value of combined detection of alpha-fetoprotein(AFP),alpha-fetoprotein heterogeneous L3 ratio(AFP-L3%)and Golgi protein 73(GP73)for primary liver cancer(PHC).Methods:A total of 86 patients with suspected PHC who were admitted to a hospital from April 2019 to April 2021 were selected.The positive situation of AFP,AFP-L3%,and GP73 in all candidates were tested.The pathological examination results were used as the gold standard to compare the levels of AFP,AFP-L3%,and GP73 between the benign lesion group and the PHC group,and analyze the application value of AFP,AFP-L3%,and GP73 alone and in combination in the diagnosis of PHC.Results:Among 86 cases of suspected PHC patients,48 cases were confirmed as PHC by pathological examination,accounting for 55.81%,and 38 cases were benign liver disease,accounting for 44.19%.The levels of AFP,AFP-L3%and GP73 in the PHC group were higher than those in the benign lesion group,and the difference was statistically significant(P<0.05).The combined detection of AFP,AFP-L3%,GP73 in the diagnosis of PHC,the sensitivity was 91.67%(44/48),the specificity was 92.11%(35/38),the accuracy was 91.86%(79/86),the positive predictive value was 93.62%(44/47),and the negative predictive value was 89.74%(35/39),which were all higher than those of AFP,AFP-L3%,and GP73 detection alone,and the difference was statistically significant(P<0.05).Conclusion:The combined detection of AFP,AFP-L3%,and GP73 in the diagnosis of PHC can improve the accuracy and sensitivity of diagnosis,and provide more reference for early diagnosis and treatment,which has high application value.
