45Hz高频振动训练干预老年膝骨关节炎患者的效果及对下肢肌肉耐力与骨密度的影响

Effects of 45Hz High-frequency Vibration Training on Elderly Patients with Knee Osteoarthritis and the Effects on Lower Extremity Muscle Endurance and Bone Density

摘要 目的:研究老年膝骨关节炎(KOA)患者使用45Hz高频振动训练的效果,及对患者骨密度和下肢肌肉耐力的影响。方法:将某院2018年7月~2020年9月期间治疗的125例老年KOA患者按抽签法分为对照组和观察组。对照组62例给予30 Hz振动训练,观察组63例给予45Hz高频振动训练,对比两组患者训练前后关节症状(西安大略和麦克马斯特大学骨关节炎指数(WOMAC)量表评分)、下肢骨密度(大转子、股骨颈、Ward三角区骨密度)、下肢肌肉耐力(踝、髋、膝屈伸耐力测试)和安全性。结果:训练前,两组患者各指标对比均无统计学意义(P>0.05),训练后,观察组患者膝、踝、髋关节伸肌和屈肌耐力较对照组高(P<0.05);观察组患者Ward三角区、股骨颈和大转子骨密度均较对照组高,WOMAC总分较对照组低(P<0.05),两组不良反应对比无统计学意义(P>0.05)。结论:45Hz高频振动训练可在安全范围内提高老年KOA患者骨密度和下肢肌肉耐力,缓解关节症状。 Objective:To study the effect of 45Hz high-frequency vibration training in elderly patients with knee osteoarthritis(KOA)and its effect on bone density and muscle endurance of lower limbs.Methods:125 elderly KOA patients treated in a hospital from July 2018 to September 2020 were divided into control group and observation group by lottery.62 cases in the control group were given 30 Hz vibration training,and 63 cases in the observation group were given 45Hz high-frequency vibration training.Joint symptoms(Western Ontario and McMaster University Osteoarthritis Index(WOMAC)Scale),bone density(greater trochanter,femoral neck,and Ward triangle bone density),muscle endurance(ankle,hip,and knee flexion and extension endurance test)and safety of lower limbs were compared between the two groups before and after training.Results:Before training,there was no statistical significance in the comparison of the two groups of patients(P>0.05).After training,the extensor and flexor endurance of knee,ankle and hip joint in the observation group were higher than those in the control group(P<0.05).The bone density of Ward triangle,femoral neck and greater trochanter in observation group was higher than that in control group,and the total WOMAC score was lower than that in control group(P<0.05).There was no statistical significance in the comparison of adverse reactions between the two groups(P>0.05).Conclusion:45Hz high-frequency vibration training can improve bone density and lower limb muscle endurance in elderly patients with KOA in a safe range,and alleviate joint symptoms.
