低体温前馈控制在全髋关节置换术中的应用及对患者血液流变学、炎症反应影响研究 被引量:1

摘要 目的:研究低体温前馈控制的实施方法及其在全髋关节置换术(THR)治疗股骨头坏死(ONFH)手术护理中的应用价值。方法:将某院收治的80例行THR治疗的ONFH患者随机分为C组和Y组各40例,手术护理中分别开展基础护理、基础护理+低体温前馈控制干预。统计两组术中、术后低体温发生率,并对比两组患者手术前后的血液流变学指标及血清炎症反应指标。结果:Y组低体温发生率低为5%,低于C组的20%(P<0.05)。术后12h,Y组患者全血高切黏度、全血低切黏度、血浆黏度均低于C组(P<0.05),Y组患者血清CRP、IL-1、TNF-α均低于C组(P<0.05)。结论:于ONFH患者行THR治疗手术护理中应用低体温前馈控制能有效预防围术期低体温的发生,改善患者血液流变学指标,降低炎症反应。 Objective:To investigate the implementation of the feedforward control of hypothermia and its application value in the surgical nursing of total hip replacement(THR)for osteonecrosis of the femeral head(ONFH).Methods:80 ONFH patients treated with THR in a hospital were randomly divided into group C(n=40)and group Y(n=40).Basic nursing and basic nursing+hypothermia feedforward control intervention were carried out in the surgical nursing,respectively.The incidence of intraoperative and postoperative hypothermia in the two groups was analyzed,and the hemorheology indexes and serum inflammatory indexes were compared between the two groups before and after surgery.Results:The incidence of hypothermia in group Y was 5%,lower than 20%in group C(P<0.05).12 hours after operation,whole blood high shear viscosity,whole blood low shear viscosity and plasma viscosity in group Y were lower than those in group C(P<0.05),and serum CRP,IL-1 and TNF-αin group Y were lower than those in group C(P<0.05).Conclusion:The application of hypothermia feedforward control in the surgical nursing of ONFH patients undergoing THR treatment can effectively prevent the occurrence of hypothermia,improve the hemorheology indexes of patients,and reduce the inflammatory response.
