摘要
目的观察电针联合新Bobath技术治疗脑卒中后肩痛的疗效。方法选择2020年1月至2021年2月南方医科大学南方医院接诊的脑卒中后肩痛患者80例作为研究对象,按照随机数表法分为观察组和对照组,每组40例。对照组患者在常规基础上联合新Bobath技术治疗,观察组患者在对照组基础上联合电针治疗,两组患者均持续治疗4周。比较两组患者治疗4周后的临床疗效,治疗前、治疗4周后视觉模拟评分法(VAS)、Fugl-Meyer运动功能量表评分及肩关节活动度的变化。结果观察组患者的临床治疗总有效率为87.50%,明显高于对照组的67.50%,差异有统计学意义(P<0.05);治疗后,观察组患者的VAS评分为(3.28±0.46)分,明显低于对照组的(4.09±0.57)分,Fugl-Meyer评分为(42.01±4.83)分,明显高于对照组的(36.59±4.21)分,差异均有统计学意义(P<0.05);治疗后,观察组患者的肩关节屈曲、外展、外旋活动度分别为(94.14±5.26)°、(88.76±4.32)°、(40.28±3.76)°,明显高于对照组的(89.32±5.07)°、(84.15±4.12)°、(37.32±3.43)°,差异均有统计学意义(P<0.05)。结论电针联合新Bobath技术治疗脑卒中后肩痛的疗效显著,值得临床推广。
Objective To study the efficacy of electroacupuncture combined with new Bobath technology on shoulder pain after stroke. Methods A total of 80 patients with shoulder pain after stroke admitted to Nanfang Hospital of Southern Medical University from January 2020 to February 2021 were selected the study objects. According to the random number table method, they were divided into an observation group and a control group, with 40 patients in each group. The patients in the control group were treated with new Bobath technologyon on the basis of routine treatment, and the patients in the observation group were combined with electroacupuncture on the basis of the control group, both 4 weeks. The clinical efficacy after 4 weeks of treatment, the changes of visual analogue scale(VAS),Fugl-Meyer motor function scale score, and shoulder joint mobility before treatment and after 4 weeks of treatment were compared between the two groups. Results The total effective rate of clinical treatment in the observation group was 87.50%, which was significantly higher than 67.50% of the control group(P<0.05). After treatment, the VAS score in the observation group was(3.28±0.46) points, which was significantly lower than(4.09±0.57) points of the control group, and Fugl-Meyer score was(42.01±4.83) points, which was significantly higher than(36.59±4.21) points of the control group(P<0.05). After treatment, the flexion, abduction and external rotation of shoulder joint in the observation group were(94.14±5.26)°,(88.76±4.32)°, and(40.28±3.76)°, which were significantly higher than(89.32±5.07)°,(84.15±4.12)°, and(37.32±3.43)° of the control group(P<0.05). Conclusion Electroacupuncture combined with new Bobath technologyon have a significant effect on post-stroke shoulder pain, which is worthy of clinical promotion.
