卡前列甲酯栓联合缩宫素预防剖宫产产后出血的效果

Effects of Carboprost methyl suppositories combined with Oxytocin in prevention of postpartum hemorrhage after cesarean section

目的:探讨卡前列甲酯栓联合缩宫素预防剖宫产产后出血的效果。方法:回顾性分析2018年8月至2020年8月该院收治的100例剖宫产产妇的临床资料,根据治疗方法不同分为对照组和研究组各50例。对照组单用缩宫素治疗,研究组在对照组基础上联合卡前列甲酯栓治疗。比较两组产后出血量、凝血指标[凝血酶原时间(PT)、纤维蛋白原(Fib)及活化部分凝血活酶时间(APTT)]水平及不良反应发生率。结果:研究组产后2、24 h出血量均少于对照组,差异有统计学意义(P<0.05);产后2 h,对照组PT、APTT较产前延长,Fib水平较产前降低,差异均有统计学意义(P<0.05);研究组凝血指标水平与产前比较,差异无统计学意义(P>0.05);研究组产后2 h PT、APTT均明显短于对照组,Fib水平明显高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:卡前列甲酯栓联合缩宫素用于剖宫产产妇,能够减少产后2、24 h出血量,保护产妇凝血功能,效果优于单用缩宫素,且安全性良好。

Objective:To explore effects of Carboprost methyl suppositories combined with Oxytocin in prevention of postpartum hemorrhage after cesarean section.Methods:100 women undergoing cesarean section in our hospital from August 2018 to August 2020 were selected as the research objects,and were divided into control group and study group according to different treatment methods,50 cases in each group.The control group was treated with single Oxytocin,while the study group was treated with Carboprost methyl suppositories on the basis of that of the control group.The postpartum blood loss,the blood coagulation index levels[prothrombin time(PT),fibrinogen(Fib)and activated partial thromboplastin time(APTT)]and the adverse reactions were compared between the two groups.Results:The 2 h and 24 h postpartum blood loss in the study group were less than those of the control group,and the differences were statistically significant(P<0.05).At 2 h postpartum,the PT and APTT of the control group were longer than those before the delivery,and the level of Fib was lower than that before the delivery(P<0.05).There was no difference in the coagulation index levels of the study group before and after the delivery.The 2 h postpartum PT and APTT levels of the study group were significantly shorter than those of the control group;the level of Fib was significantly higher than that of the control group;and the differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Carboprost methyl suppositories combined with Oxytocin for the patients after cesarean section can reduce the 2 h and 24 h postpartum blood loss and protect the coagulation function with good safety.Moreover,it is superior to single Oxytocin.
