经脐单孔腹腔镜阑尾切除术治疗穿孔性阑尾炎患儿的效果

Effects of transumbilical single port laparoscopic surgery in treatment of children with perforating appendicitis

目的:观察经脐单孔腹腔镜阑尾切除术治疗穿孔性阑尾炎患儿的效果。方法:选取80例穿孔性阑尾炎患儿作为研究对象,按照随机数字表法分为观察组与对照组各40例。对照组采用传统三孔腹腔镜阑尾切除术治疗,观察组采用经脐单孔腹腔镜阑尾切除术治疗,比较两组手术时间、术中出血量、住院时间、肠鸣音恢复时间、首次排气时间、首次排便时间和并发症发生率。结果:观察组手术时间和住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);观察组肠鸣音恢复时间、首次排气时间、首次排便时间均短于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为7.50%,低于对照组的32.50%,差异有统计学意义(P<0.05)。结论:经脐单孔腹腔镜阑尾切除术治疗穿孔性阑尾炎患儿可缩短手术时间、住院时间、肠鸣音恢复时间、首次排气时间和首次排便时间,降低术中出血量和并发症发生率,效果优于传统三孔腹腔镜阑尾切除术。

Objective:To observe effects of transumbilical single port laparoscopic surgery in treatment of children with perforating appendicitis.Methods:80 children with perforating appendicitis were selected as the research objects,and were divided into observation group and control group according to the random number table method,40 cases in each group.The control group was treated with traditional three-port laparoscopic appendectomy,while the observation group was treated with transumbilical single-port laparoscopic appendectomy.The operation time,the intraoperative blood loss,the hospitalization time,the bowel sound recovery time,the first exhaust time,the first defecation time,and the complication rate were compared between the two groups.Results:The operation time and the hospitalization time of the observation group were shorter than those of the control group;the intraoperative blood loss was lower than that of the control group;and the differences were statistically significant(P<0.05).The bowel sound recovery time,the first exhaust time,and the first defecation time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was 7.50%,which was lower than 32.50%in the control group,and the difference was statistically significant(P<0.05).Conclusions:Transumbilical single port laparoscopic appendectomy in the treatment of the children with perforating appendicitis can shorten the operation time,the hospitalization time,the bowel sound recovery time,the first exhaust time,and the first defecation time,and reduce the intraoperative blood loss and the complication rate.Moreover,it is superior to traditional three-port laparoscopic appendectomy.
