磁性附着体与卡环式义齿在老年牙列缺损修复患者中的应用效果比较

Comparison of application effects of magnetic attachments and clasp dentures in elderly patients with dentition defect restoration

目的:比较磁性附着体与卡环式义齿在老年牙列缺损修复患者中的应用效果。方法:回顾性分析2017年12月至2020年12月在该院行口腔修复的120例老年牙列缺损患者的临床资料,根据修复方式不同分为对照组和研究组各60例。对照组采用传统卡环式修复,研究组采用磁性附着体修复,比较两组修复后牙周指标[菌斑指数(PLI)、龈沟出血指数(SBI)、牙周探诊深度(PD)和临床附着丧失(CAL)]水平、修复前后龈沟液成分[肿瘤坏死因子(TNF-α)、C反应蛋白(CRP)、白细胞介素-6(IL-6)、碱性磷酸酶(ALP)、基质金属蛋白酶-2(MMP-2)]水平变化情况、牙齿功能评分和并发症发生率。结果:修复后,研究组PLI、SBI、PD和CAL均低于对照组,差异有统计学意义(P<0.05);研究组TNF-α、CRP、IL-6、ALP、MMP-2水平均低于对照组,差异有统计学意义(P<0.05);研究组舒适度、咀嚼功能、语言功能和牙齿美观度评分均高于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:磁性附着体应用于老年牙列缺损修复患者,可降低牙周指标水平和龈沟液炎性成分水平,提高牙齿功能评分,优于传统卡环式修复效果。

Objective:To compare application effects of magnetic attachments and clasp dentures in elderly patients with dentition defect restoration.Methods:A retrospective analysis of the clinical data of 120 elderly patients with dentition defect who underwent oral restoration in the hospital from December 2017 to December 2020 was performed.According to different restoration methods,they were divided into control group and study group,each with 60 cases.The control group used traditional clasp restoration,while the study group used magnetic attachment restoration.The levels of periodontal indexes[plaque index(PLI),gingival crevicular bleeding index(SBI),periodontal probing depth(PD)and clinical attachment loss(CAL)],the gingival crevicular fluid composition level changes before and after the restoration[tumor necrosis factor(TNF-α),C-reactive protein(CRP),interleukin-6(IL-6),alkaline phosphatase(ALP),matrix metalloproteinase-2(MMP-2)],the teeth function score and the complication rate were compared between the two groups.Results:After the restoration,the PLI,SBI,PD and CAL levels of the study group were lower than those of the control group,and the differences were statistically significant(P<0.05).The levels of TNF-α,CRP,IL-6,ALP,and MMP-2 in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).The scores of comfort,masticatory function,language function and tooth aesthetics in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).However,there was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:Magnetic attachments for the elderly patients with dentition defect restoration can reduce the levels of periodontal indexes and the levels of inflammatory components in the gingival crevicular fluid,and improve the tooth function score.Moreover,it is superior to traditional clasp restoration.
