集束化护理在预防NICU呼吸机相关性肺炎中的应用效果

Application effects of cluster nursing in prevention of ventilator-associated pneumonia in NICU

目的:观察集束化护理在预防新生儿重症监护室(NICU)呼吸机相关性肺炎中的应用效果。方法:选取90例NICU机械通气患儿为研究对象,按照随机数字表法分为观察组与对照组各45例。对照组行常规护理,观察组行集束化护理,比较两组呼吸机相关性肺炎发生率、机械通气次数、机械通气时间、住院时间和患儿家长护理满意度。结果:观察组呼吸机相关性肺炎发生率为4.44%,明显低于对照组的17.78%,差异有统计学意义(P<0.05);观察组机械通气次数少于对照组,机械通气时间和住院时间均短于对照组,差异有统计学意义(P<0.05);观察组患儿家长护理满意度为95.56%,明显高于对照组的80.00%,差异有统计学意义(P<0.05)。结论:集束化护理应用于NICU机械通气患儿可降低呼吸机相关性肺炎发生率,减少机械通气次数,缩短机械通气时间和住院时间,以及提高患儿家长护理满意度,效果优于常规护理。

Objective:To observe application effects of cluster nursing in prevention of ventilator-associated pneumonia in neonatal intensive care unit(NICU).Methods:90 children with mechanical ventilation in NICU were selected as the research objects,and were divided into observation group and control group according to the random number table method,45 cases in each group.The control group received routine nursing,while the observation group received the cluster nursing.The incidence of ventilator-associated pneumonia,the number of mechanical ventilation,the mechanical ventilation time,the hospitalization time and the satisfaction degree of the parents of the children were compared between the two groups.Results:The incidence of ventilator-associated pneumonia in the observation group was 4.44%,which was significantly lower than t 17.78%in the control group,and the difference was statistically significant(P<0.05).The number of mechanical ventilation in the observation group was less than that in the control group;the mechanical ventilation time and the hospitalization time were shorter than those in the control group;and the differences were statistically significant(P<0.05).Further,the nursing satisfaction degree of the parents of the children in the observation group was 95.56%,which was significantly higher than 80.00%in the control group,and the difference was statistically significant(P<0.05).Conclusions:The application of cluster nursing for the ventilator-associated pneumonia in children in NICU can reduce the incidence of ventilator-associated pneumonia,reduce the number of mechanical ventilation,shorten the mechanical ventilation time and the hospitalization time,and increase the satisfaction degree of the parents of these children.Moreover,it is superior to conventional nursing.
