介入导管碎栓联合低剂量静脉溶栓治疗急性肺血栓栓塞症患者的效果

Effects of interventional catheter fragmentation combined with low-dose intravenous thrombolysis in treatment of patients with acute pulmonary thromboembolism

摘要 目的:观察介入导管碎栓联合低剂量静脉溶栓治疗急性肺血栓栓塞症患者的效果。方法:选取90例急性肺血栓栓塞症患者为研究对象,按照抽签法分为对照组和研究组各45例。对照组采用静脉溶栓治疗,研究组采用介入导管碎栓联合低剂量静脉溶栓治疗,比较两组治疗前后血气指标[血氧饱和度(SaO_(2))、动脉血氧分压(PaO_(2))]水平、临床疗效和并发症发生率。结果:治疗后,两组SaO_(2)、PaO_(2)水平均高于治疗前,差异有统计学意义(P<0.05),但组间比较,差异无统计学意义(P>0.05);研究组治疗总有效率为91.11%(41/45),高于对照组的71.11%(32/45),差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:介入导管碎栓联合低剂量静脉溶栓治疗急性肺血栓栓塞症患者可提高治疗总有效率,效果优于静脉溶栓治疗。 Objective:To observe effects of interventional catheter fragmentation combined with low-dose intravenous thrombolysis in treatment of patients with acute pulmonary thromboembolism.Methods:90 patients with acute pulmonary thromboembolism were selected as the research objects,and were divided into control group and study group according to the lottery method,45 cases in each group.The control group was treated with intravenous thrombolysis,while the study group was treated with interventional catheter fragmentation combined with low-dose intravenous thrombolysis.The levels of blood gas indexes[blood oxygen saturation(SaO_(2)),arterial partial pressure of oxygen(PaO_(2))],the clinical efficacy and the complication rate were compared between the two groups before and after the treatment.Results:After the treatment,the levels of SaO_(2) and PaO_(2) in the two groups were higher than those before the treatment,and the differences were statistically significant(P<0.05),but there was no statistically significant difference between the groups(P>0.05).The total effective rate of treatment in the study group was 91.11%(41/45),which was higher than 71.11%(32/45)in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:Interventional catheter fragmentation combined with low-dose intravenous thrombolysis in the treatment of the patients with acute pulmonary thromboembolism can improve the total effective rate of treatment.Moreover,it is superior to intravenous thrombolysis.
