改良粘接固位术在后牙缺损患者种植义齿修复中的应用效果

摘要 目的:观察改良粘接固位在后牙缺损患者种植义齿修复中的应用效果。方法:选取行后牙种植单冠修复的60例患者作为研究对象,按照随机数字表法分为观察组和对照组各30例。对照组采用传统粘接固位术治疗,观察组采用改良粘接固位术治疗。比较两组术后7 d菌斑指数(PLI)、龈沟出血指数(SBI)、牙周探诊深度(PD)和牙龈乳头指数(PIS),术前及术后30 d咀嚼效能(ME)和龈沟液白细胞介素(IL)-2、IL-8水平,以及并发症发生率。结果:术后7 d,观察组PLI、SBI和PIS均低于对照组,PD小于对照组,差异均有统计学意义(P<0.05);术后30 d,观察组ME高于对照组,IL-2、IL-8水平均低于对照组,差异有统计学意义(P<0.05);两组均未发生明显并发症。结论:改良粘接固位术应用于后牙缺损患者种植义齿修复中,可改善术后牙菌斑和出血情况,减轻炎症反应,提升咀嚼效能,效果优于传统粘接固位术。 Objective:To observe application effects of modified adhesive retention in implant denture restoration in patients with posterior teeth defects.Methods:60 patients who underwent posterior dental implant single crown restoration were selected as the research objects,and were divided into observation group and control group according to the random number table method,30 cases in each group.The control group was treated with traditional adhesive retention,while the observation group was treated with modified adhesive retention.The levels of plaque index(PLI),sulcus bleeding index(SBI),periodontal probing depth(PD)and papillary index score(PIS)7 days after the surgery,the masticatory functions before and 30 days after the surgery,the levels of gingival crevicular fluid interleukin(IL)-2 and IL-8,and the incidence of complications were compared between the two groups.Results:7 days after the surgery,the levels of PLI,SBI and PIS of the observation group were lower than those of the control group;the PD level was lower than that of the control group;and the differences were statistically significant(P<0.05).30 days after the surgery,the ME level in the observation group was higher than that in the control group;the IL-2 and IL-8 levels were lower than those in the control group;and the differences were statistically significant(P<0.05).No obvious complications occurred in the two groups.Conclusions:The modified adhesive retention used in the implant denture restoration in the patients with posterior teeth defects can improve the postoperative dent al plaque and bleeding conditions,relieve the inflammation reac tions,and improve the masticatory efficiency.Moreover,it is superior to traditional adhesive retention.
