补肺健脾方合六君子汤治疗急性加重期慢性阻塞性肺疾病患者的效果

Effects of Bufei Jianpi recipe and Liujunzi decoction in treatment of chronic obstructive pulmonary disease in acute exacerbation period

目的:观察补肺健脾方合六君子汤治疗急性加重期慢性阻塞性肺疾病(COPD)患者的效果。方法:选取96例急性加重期COPD患者作为研究对象,按照随机数字表法分为对照组和观察组各48例。对照组采取常规西药治疗,观察组在对照组基础上给予补肺健脾方合六君子汤治疗。比较两组临床疗效,治疗前后中医证候积分、肺功能指标[第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)、FEV_(1)/FVC]水平、炎性因子[血清肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)]水平和不良反应发生率。结果:观察组治疗总有效率为91.67%,高于对照组的75.00%,差异有统计学意义(P<0.05);治疗后,观察组主症、次症等中医证候积分均低于对照组,差异有统计学意义(P<0.05);观察组FVC、FEV_(1)、FEV_(1)/FVC水平均高于对照组,TNF-α、IL-8水平均低于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率为4.17%,低于对照组的18.75%,差异有统计学意义(P<0.05)。结论:在常规西药治疗基础上给予补肺健脾方合六君子汤治疗急性加重期COPD患者,可提高治疗总有效率和肺功能指标水平,降低中医证候积分、炎性因子水平和不良反应发生率,其效果优于单纯常规西药治疗。

Objective:To observe effects of Bufei Jianpi recipe and Liujunzi decoction in treatment of chronic obstructive pulmonary disease(COPD)in acute exacerbation period.Methods:96 patients with COPD in acute exacerbation period were selected as the research objects,and divided into control group and observation group according to the random number table method,48 cases in each.The control group was treated with conventional western medicine,while the observation group was treated with Bufei Jianpi recipe and Liujunzi decoction on the basis of that of the control group.The clinical efficacy,the TCM syndrome scores before and after the treatment,the lung function index levels[forced expiratory volume in 1 second(FEV_(1)),forced vital capacity(FVC),FEV_(1)/FVC],the inflammatory factors[serum tumor necrosis factor-α(TNF-α),interleukin-8(IL-8)]levels and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of treatment in the observation group was 91.67%,which was higher than 75.00%in the control group,and the difference was statistically significant(P<0.05).After the treatment,the scores of TCM syndromes such as primary and secondary symptoms in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of FVC,FEV_(1) and FEV_(1)/FVC of the observation group were higher than those of the control group;the levels of TNF-αand IL-8 were lower than those of the control group;and the differences were statistically significant(P<0.05).Further,the incidence of adverse reactions in the observation group was 4.17%,which was lower than 18.75%in the control group,and the difference was statistically significant(P<0.05).Conclusions:On the basis of the conventional western medicine treatment,Bufei Jianpi recipe combined with Liujunzi decoction in the treatment of the patients with COPD in acute exacerbation period can increase the total effective rate of treatment and the levels of pulmonary function indicators,and reduce the TCM syndrome score,the levels of inflammatory factors and the incidence of adverse reactions.Moreover,it is superior to conventional western medicine treatment.
