摘要 目的:评价自行研制的Lv-6型真肝模拟人在创新手术磁锚定减戳孔腹腔镜胆囊切除术培训中的应用效果。方法:将18名不同级别的学员随机分为实验组与对照组,学员均按既定计划完成理论学习与模拟培训(实验组使用Lv-6型真肝模拟人、对照组使用常规箱式腹腔镜模拟训练器),然后在实验猪上完成磁锚定减戳孔腹腔镜胆囊切除术,根据手术完成情况评价其学习、培训效果。结果:18例学员均按计划完成理论学习,在模拟培训中,实验组学员对Lv-6真肝模拟人评价均为良好,对照组对箱式腔镜模拟训练器评价较差。在实验猪磁锚定减戳孔腹腔镜胆囊切除术中,实验组均成功完成任务,对照组仅1组完成手术,另外两组因术中出现严重的血管、胆道损伤被迫中止手术。实验组手术时间、手术流畅性优于对照组。结论:Lv-6型真肝模拟人较常规箱式腹腔镜模拟训练器在磁锚定腹腔镜胆囊切除术培训中能获得更佳的培训效果。 Objective:To evaluate the effect of self-developed Lv-6 real liver simulator in the training of magnetic anchor less-Trocar laparoscopic cholecystectomy.Methods:A total of 18 students with different levels were randomly divided into the study group and the control group.All the students completed the theoretical learning and simulation training according to the established plan(the students of study group used Lv-6 real liver simulator,and the students of control group used conventional box training laparoscopic simulator),and then completed the magnetic anchor less-Trocar laparoscopic cholecystectomy on the experimental pigs.The effect of learning and training was evaluated according to the operation completion.Results:All students completed the theoretical study as planned.In the simulation training,the students in the study group had good evaluation of Lv-6 real liver simulator,while the students in the control group had poor evaluation of box training laparoscopic simulator.In the magnetic anchor less-Trocar laparoscopic cholecystectomy of experimental pigs,all the students in study group successfully completed the operation task,while only 1 control group completed the operation,and the other two control groups were forced to abort the operation due to severe vascular and biliary tract injuries during the operation.In terms of operation time and surgical fluency,the study group was significantly better than the control group.Conclusions:Compared with conventional box training laparoscopic simulator,Lv-6 real liver simulator can obtain better training effect in magnetic anchor less-Trocar laparoscopic cholecystectomy.
