数字减影血管造影下神经介入溶栓术联合静脉溶栓治疗急性缺血性卒中大动脉闭塞型患者中的效果评价

Evaluation of the effect of digital subtraction angiography lower nerve interventional thrombolysis combined with intravenous thrombolysis in patients with acute ischemic stroke with large artery occlusion

目的探讨数字减影血管造影(DSA)下神经介入溶栓术联合静脉溶栓治疗急性缺血性卒中大动脉闭塞型患者的临床效果。方法选取本院2018年7月至2020年7月收治的急性缺血性卒中大动脉闭塞型患者80例,按照随机数字表法分成两组,各40例。对照组单纯进行静脉溶栓治疗,观察组在对照组基础上联合DSA下神经介入溶栓术治疗,比较两组即刻血管再通情况、临床疗效及并发症发生率。结果观察组即刻血管再通率(60%)高于对照组(30%),差异有统计学意义(P<0.05)。观察组治疗总有效率(50.00%)高于对照组(20.00%),差异有统计学意义(P<0.05)。治疗后,观察组美国独立卫生研究院卒中量表(NIHSS)评分明显低于对照组,差异有统计学意义(P<0.05)。两组并发症发生率比较差异无统计学意义。结论采取DSA下神经介入溶栓术联合静脉溶栓治疗ICVD患者效果显著,能提高血管再通率,加速神经功能恢复,安全性高,值得临床推广应用。

Objective To investigate the clinical effect of digital subtraction angiography(DSA)lower nerve interventional thrombolysis combined with intravenous thrombolysis in the treatment of patients with acute ischemic stroke with aortic occlusion.Methods A total of 80 patients with acute ischemic stroke and large artery occlusion in our hospital from July 2018 to July 2020 were selected and divided into two groups according to the random number table method,with 40 cases in each group.The control group received intravenous thrombolysis,the observation group was combined with DSA nerve interventional thrombolysis on the basis of the control group.The immediate recanalization,clinical efficacy and complications were compared between the two groups.Results The immediate vascular recanalization rate of the observation group(60%)was higher than that of the control group(30%),the difference was statistically significant(P<0.05).The total effective rate of treatment in the observation group(50.00%)was higher than that in the control group(20.00%),the difference was statistically significant(P<0.05).After treatment,the scores of the Independent Institutes of Health Stroke Scale(NIHSS)of the observation group was significantly lower than that of the control group(P<0.05).There was no significant difference in the incidence of complications between the two groups.Conclusion For patients with ICVD,the combination of DSA nerve interventional thrombolysis and intravenous thrombolysis has good effect,can improve the blood vessel to accelerate the recovery of nerve function,promote the recanalization of the occluded blood vessel,it has high safety and is worthy of clinical promotation.
