艾司氯胺酮对开胸手术患者应激反应的影响 被引量:5

摘要 目的探讨艾司氯胺酮对开胸手术患者应激反应的影响。方法选取本院2020年1—12月收治的需进行开胸手术的患者50例,随机分为两组,各25例。对照组采用常规麻醉方案,观察组在对照组基础上采用艾司氯胺酮进行辅助麻醉。比较两组麻醉效果及手术期间应激反应情况。结果手术30 min、术后12 h,观察组心率均低于对照组,平均动脉压均高于对照组,差异有统计学意义(P<0.05);手术30 min和术后12 h,观察组血清皮质醇、血管紧张素水平均低于对照组,差异有统计学意义(P<0.05);术后,观察组各阶段疼痛程度评分均低于对照组,差异有统计学意义(P<0.05)。结论针对需进行开胸手术的患者在手术治疗过程中合理运用艾司氯胺酮进行麻醉,能有效减少患者在手术过程中所产生的应激反应,对于保证手术的成功及帮助患者术后恢复等均有重要意义。 Objective To analyze the effect of esmketamine on stress response in patients undergoing thoracotomy.Methods 50 patients undergoing thoracotomy who were admitted to our hospital from January to December 2020 were selected and they were randomly divided into two groups,with 25 cases in each group.The control group used conventional anesthesia,while the observation group used the aid of esketamine for anesthesia on the basis of the control group.The anesthesia effect and stress response during operation were compared between the two groups.Results At 30 minutes after operation and 12 hours after operation,the heart rate of the observation group was lower than that of the control group,and the average arterial pressure was higher than that of the control group,the difference was statistically significant(P<0.05);30 minutes after operation and 12 hours after operation,the observation group serum cortisol and angiotensin levels were lower than those of the control group,the difference was statistically significant(P<0.05);after the operation,the pain degree scores at all stages of the observation group were lower than the control group,the difference was statistically significant(P<0.05).Conclusion The reasonable use of esmketamine for anesthesia can effectively reduce the stress response of patients during the operation,which is of great significance to ensure the success of the operation and help patients recover quickly after the operation.
