甲状腺功能减退合并阻塞性睡眠呼吸暂停低通气综合征的相关性分析

Correlation Analysis of Hypothyroidism with Obstructive Sleep Apnea Hypopnea Syndrome

目的:探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)伴发甲状腺功能减退症(简称甲减)的临床特征,进而为临床有效诊断、治疗提供参考。方法:选取2019年6月至2020年6月山东第一医科大学附属肥城医院收治的OSAHS患者52例作为研究对象,深入分析其临床资料。结果:OSAHS好发于男性,52例患者中,伴发甲减者19例。通过多导睡眠监测(PSG)显示,睡眠呼吸暂停低通气指数(AHI)9~65次/h,平均睡眠呼吸暂停低通气指数(33.5±16.2)次/h。27例重症OSAHS,伴发甲减10例;20例中度OSAHS,伴发甲减7例;5例轻度OSAHS,伴发甲减2例。血氧饱和度44%~82%,平均血氧饱和度(66.6±11.5)%。结论:OSAHS伴发甲减患者病情十分严重,且两病特征相似,需要临床加强鉴别,及时给予综合治疗。

Objective:To investigate the clinical characteristics of obstructive sleep apnea hypopnea syndrome(OSAHS)with hypothyroidism,so as to provide references for effective diagnosis and treatment.Methods:A total of 52 cases of OSAHS patients admitted to Feicheng Hospital Affiliated to Shandong First Medical University from June 2019 to June 2020 were selected as the research objects,and their clinical data were analyzed in depth.Results:OSAHS was most common in males.Among the 52 patients,19 were accompanied by hypothyroidism.The polysomnography(PSG)monitoring showed that the apnea hypopnea index(AHI)was 9~65 times per hour,with an average of(33.5±16.2)times/h.There were 27 patients with severe OSAHS,including 10 patients with hypothyroidism.There were 20 patients with moderate OSAHS and 7 patients with hypothyroidism.There were 5 patients with mild OSAHS,including 2 patients with hypothyroidism.The oxygen saturation ranged from 44%to 82%,with an average of(66.6±11.5)%.Conclusion:OSAHS patients with concomitant hypothyroidism are very serious,with similar characteristics of the two diseases,which require strengthened clinical differentiation and timely comprehensive treatment before recovery.
